Background: Identifying risk factors of breast cancer is a key point for preventive strategies to reduce the incidence. The aim of current study was to determine most important risk factors for breast cancer in the Eastern Mediterranean Region (EMR) using a systematic review. Materials and Methods: PubMed, Scopus, Web of Science till August 24, 2012 and the reference lists of all included studies were searched. Analytic studies which had reported odds ratios (OR), relative risk (RR) or required data to calculate them were included. A total of 343 studies were critically appraised and finally 30 studies were meta-analyzed. Heterogeneity between the studies was assessed by I² and Cochran's Q. Egger's test was used to assess publication bias. Results: Twenty five casecontrol studies, one nested case-control and four cohort studies were included. The largest ORs were obtained for history of no live birth (2.25; 95%CI: 1.58-3.18), body mass index (BMI) more than 30 (2.21; 95%CI: 1.71-2.36), age at first pregnancy more than 30 years old (1.52; 95%CI: 1.30-1.77) and meat consumption more than three times per week (1.39; 95%CI: 1.03-1.87). The other important predictors were higher education and smoking as risk factors, physical activity and ovulatory stimulating medication as protective factors. Conclusions: The most important predictors of breast cancer in EMR were history of no live birth, BMI more than 30, age at first pregnancy more than 30 years old, physical inactivity and smoking. Almost all these risk factors are consistent with known risk factors for this cancer in other parts of the world.

Olive oil and risk of breast cancer: a systematic review and dose–response meta-analysis of observational studies

Olive oil consumption has been suggested to be inversely associated with breast cancer risk, likely due to its high monounsaturated fatty acid and polyphenol content. The purpose of this meta-analysis was to assess the association between olive oil and breast cancer risk, including assessing the potential for a dose-response association. We performed a systematic search of PubMed, Web of Science, CINAHL, and Cochrane Central Register of Controlled Trials (CENTRAL) through June 2020, identifying 10 observational studies (two prospective studies and 8 case-control studies) for inclusion in the present meta-analysis. We estimated summary odds ratios (OR) and 95% confidence intervals (CI) for the highest versus lowest olive oil intake category across studies using random effect models, and assessed the dose-response relationship between olive oil and breast cancer risk using restricted cubic splines. The summary OR comparing women with the highest intake to those with the lowest category of olive oil intake was 0.48 (95% CI = 0.09-2.70) in prospective studies and 0.76 (95% CI=0.54-1.06) in case-control studies, with evidence of substantial study heterogeneity (prospective: I2 = 89%, case-control: I2 = 82%). There was no significant dose-response relationship for olive oil and breast cancer risk; the OR for a 14 g/day increment was 0.93 (95% CI=0.83 - 1.04). There may be a potential inverse association between olive oil intake and breast cancer; however, since the estimates are non-significant and the certainty level is very low, additional prospective studies with better assessment of olive oil intake are needed.

Introduction: There is evidence that breast cancer patients have impaired immune function, which can promote cancer progression. Chronic inflammation is involved in all stages of carcinogenesis. Studies have shown that n-3 polyunsaturated fatty acids eicosapentaenoic (EPA) and docosahexaenoic (DHA) acids have immunomodulatory and anti-inflammatory properties and are useful in the prevention and treatment of various diseases, including cancer. In this context, this study aimed to investigate in patients with newly diagnosed breast cancer the effects of fish oil supplementation in lymphocytes subpopulation, proinflammatory cytokines, C-reactive protein (CRP) and prostaglandin E 2. Methodology: A randomized, placebo controlled, double-blind clinical trial was conducted in patients diagnosed with breast cancer, divided into two groups (fish oil group-GO and placebo group-GP). The GO patients received 2 g/ day of fish oil concentrate containing 1.8 g of n-3 fatty acids. The placebo group received two capsules/day of 1 g of mineral oil with similar appearance to fish oil. All patients were supplemented for four weeks during the period prior to surgical treatment. Before and at the end of supplementation, we collected anthropometric, body composition and dietary intake data and blood samples for lymphocyte phenotyping, laboratory tests (fasting glucose, serum albumin, lipid profile), serum fatty acids analysis and inflammatory markers (IL-1β, IL-6, TNF-α, CRP and prostaglandin E2). Statistical analysis was performed using parametric and nonparametric tests to compare the results of both groups. Results: GO consisted of 18 patients and the GP 19. At baseline no significant differences were observed between the groups in terms of socioeconomic, anthropometric and clinicopathological parameters. Most of the patients had infiltrating ductal carcinoma and 55.6% and 57.9% of GO and GP, respectively, had negative HER2 tumor. After the intervention there was a significant increase in the amount of EPA (p = 0.004), DHA (p = 0.007) and the total of n3 fatty acids (p = 0.004) in plasma phospholipid of the GO patients. There was a reduction in the n6/ n3 ratio (p = 0.002). In GP patients there was a reduction in the percentage of peripheral blood CD4+ T lymphocytes counts and increased plasma C-reactive protein concentration. In GO patients The proportion of peripheral blood CD4 + T lymphocytes and serum of CRP levels were maintained. There were no significant changes in serum levels of proinflammatory cytokines and prostaglandin E 2. Conclusion: Supplementation of treatment naïve breast cancer patients with 1.8 g/ day of n-3 fatty acids for 4 weeks caused a significant change in the composition of plasma fatty acids, kept the frequency of CD4+ T cells and serum CRP, suggestive of a beneficial effect of the supplement on patient's immune system.
