Vesicovaginal Fistula (VVF), an abnormal communication between the urinary bladder and vagina, is one of the most distressing and embarrassing health problem for the ladies. The present observational study was planned to inquire into the demographic and aetiologic pattern of vesicovaginal fistula and the long-term success rate of its surgical management following different techniques of repair in a tertiary care centre of West Bengal, India. MATERIAL AND METHODSA total of 47 patients with vesicovaginal fistula presented at our hospital over a span of nine years, were observed during their course of treatment. The patients were evaluated with clinical history, physical examination, routine laboratory investigations, intravenous urogram and cystoscopy. Then the patients had undergone operation by vaginal or abdominal route. Patients were discharged from the hospital with per urethral catheter. On 21 st post-operative day, routine cystogram was done in every patient before catheter removal to exclude the failure of the operation. RESULTOf the observed 47 patients, 66% were tracked back to their obstetric causes and 34% patients could be linked up to gynaecological aetiology like a complication of hysterectomy or after brachytherapy for carcinoma of cervix. In 23.4% of patients, the repair of fistula was done by vaginal route, whereas in 76.6% cases by abdominal approach. Overall success rate of surgical repair was 87.3%, which is comparable to the success rate mentioned in literature. CONCLUSIONIn spite of a decline in the incidence of vesicovaginal fistula in the western world, it is still highly prevalent in the developing countries. Prolonged obstructive labour was found as the most common aetiology of this devastating condition in our region. Timely intervention with meticulous surgical technique is essential for an acceptable success rate in fistula repair surgery. However, improved obstetric care, institutional delivery, high literacy rate, prevention of early marriage-all are of paramount importance for the prevention of occurrence of this socio-medical problem.

Primary lymphoma of the urinary bladder is exceedingly rare, representing 0.2% of all extranodal non-Hodgkin's lymphoma. Although Matsuno et al. and others state the most common type is mucosa-associated lymphoid tissue (MALT) lymphoma, 20% of all the primary lymphomas of the urinary bladder are considered to be high grade neoplasms; the majority being diffuse large B-cell lymphoma (DLBCL). This is a case report of a 48-year-old man that presented with hematuria, frequency, nocturia, and flank pain that was found to have high grade DLBCL. Twenty-six other cases of both low and high grade primary bladder lymphomas were selected in order to provide a thorough comparison of different treatment modalities. Of the cases reviewed, bladder lymphoma was more common in females (2:1). The average age at diagnosis was 63.9 years old (low grade: 68.7 years old, high grade: 58.8 years old). The most common low-grade neoplasm was MALT lymphoma (85.7%). For the low-grade malignancies, the most successful treatments were simple therapies (2 transurethral resection of a bladder tumour [TURBT], 1 antibiotics), solitary chemotherapy, and combination TURBT/chemo; all 3 of which achieved 100% clinical remission (CR) in the cases reviewed. The most common high grade neoplasm was DLBCL (76.9%). The most successful therapies used to treat high grade lesions were solitary chemotherapy (cyclophosphamide, duanorubacin, vincristine, prednisolone [CHOP] or ritoximab, CHOP [R-CHOP]) and combination therapies (2 radiation/CHOP, 2 surgery/CHOP). In the agreement with the current literature, this review has shown that simple therapies (TURBT) are equally as effective as aggressive treatments (chemotherapy, radiation) and should therefore be used as first line treatment in low grade tumors. For high grade malignancies, chemotherapy (R-CHOP or CHOP) alone or combination therapy (CHOP/surgery or CHOP/radiation) is recommended.

Rare cases of VVF have been reported after illegal abortion. 2 and vigorous sexual activities. 3 One rare case of VVF due to primary lymphoma of urinary bladder has been reported by Evans et al. 4 The physical and psychological impact of constant urinary incontinence from a VVF can be overwhelming because of the burden of continual wetness, undesirable odors, vaginal and bladder infections, and related discomfort. Despite advances in the management, VVF repair remains technically challenging. The success rate ABSTRACT Background: Vesicovaginal Fistula (VVF), an abnormal communication between the urinary bladder and vagina, is one of the most distressing and embarrassing health problems for the females. The present retrospective study was done to analyse the demographic and aetiologic pattern of vesicovaginal fistula and to ascertain the outcomes of its surgical management following different techniques of repair in a tertiary care centre. Methods: Retrospective analysis of case records of 24 cases treated between January 2015 to July 2017 at Department of Urology KIMS, Hubli was done. Duration of symptoms, mechanism of injury, radiological and biochemical reports, cystoscopic recordings of patients and operative records were analysed. Results: Mean age was 40.8 years (range 20-70 years). Fistula size varied from 0.5-5cm. location of the fistula was supratrigonal in 20 cases (84%) and trigonal in 4 cases (16%). Fitula was simple in 16 cases (68%) and complex in 8cases (32%). 21 cases were treated by open surgical method and laparoscopic technique in 3 cases. VVF repair was done by abdominal approach in 17 (67%) cases and by vaginal approach in 4 (33%) cases. Cases were followed for 2 -25 months. success rate of 100% (3 cases) who were operated laparoscopically and 17/21 (80.95%) in patients treated by open surgical method was noted. Failures in present study could be attributable to extensive fibrosis especially in prior failed repairs and in those with prolonged duration of symptoms. Conclusions: In the present study complex fistulas had higher failure rate. The principles of fistula repair, experience of surgeon and case selection remain important determinants of outcome.
