The feasibility of scarless single-port transumbilical total laparoscopic hysterectomy: initial clinical experience

Abstract: SPLS is a feasible approach for hysterectomy in terms of operative time, complication rates, and cosmetic results. However, the possible benefits for patients such as better cosmetic outcomes, reduced pain, and lower complication rates should be evaluated in randomized prospective studies.

“…Many attempts have been made to reduce complications by minimizing the number of trocars inserted and reducing the size of incisions, and have been applied to cholecystectomy, appendectomy and splenectomy using single port laparoscopic surgery (SPLS) [7][8][9]. SPLS has also been used to perform hysterectomies, with acceptable clinical safety [10,11]. However, there are few reports assessing the feasibility of SPLS for the treatment of adnexal lesions.…”

“…For the SPLS patients, a 1.2-cm vertical intraumbilical skin incision was made, and a 1.5-cm rectus fasciotomy was performed for entrance to the peritoneal cavity. The authors made a single three-channel port using the Alexis wound retractor (Applied Medical, Rancho Santa Margarita, CA) and a surgical glove, as reported in a previous study [7].…”

“…The authors demonstrated in a previous report that SPLS is a feasible approach for total laparoscopic hysterectomy (TLH) in terms of surgical outcomes [7]. Despite the surgical feasibility, technical difficulty performing hysterectomy remains an obstacle to the widespread use of SPLS.…”
