Behcet's disease (BD) is a chronic systemic inflammatory disease. Inflammatory reactions trigger the oxidative stress and oxidants decrease the level of antioxidants. In the present study, we aimed to determine serum oxidative/antioxidative status in patients with BD. Serum antioxidative status was evaluated by measuring total antioxidant capacity (TAC), paraoxonase 1, arylesterase, sulfhydryl groups and ceruloplasmin in patients with BD and in healthy controls. Serum oxidative status was evaluated by measuring total peroxide (TP), lipid hydroperoxides and oxidative stress index (OSI). OSI was calculated by percent ratio of TP to TAC. Serum levels of TAC, sulfhydryl groups and activities of paraoxonase 1, arylesterase, and ceruloplasmin were significantly lower in patients than in controls ( p < 0.001 for all). In contrast, TP, lipid hydroperoxides and OSI values were significantly higher in patients than in controls ( p < 0.001 for all). Further, the level of TAC was negatively correlated with the levels of TP, lipid hydroperoxides and OSI both in the BD (r = −0.578, p < 0.01; r = −0.559, p < 0.01 and r = −0.552, p < 0.01, respectively) and the control groups (r = −0.469, p < 0.05; r = −0.351, p < 0.05 and r = −0.391, p < 0.05, respectively). These results indicate that the oxidant parameters increased and antioxidant parameters decreased in patients with BD; therefore, these patients might have been exposed to oxidative stress. We suggest that impaired oxidant/ antioxidant balance should be taken into consideration in the follow-up of patients with BD.

Objective This study was undertaken to elucidate the pathogenesis and heterogeneity of Behçet's disease (BD) involving different organs using in‐depth proteomics to identify the biomarkers for clinical assessment and treatment of patients with BD. Methods We measured the expression levels of proteins in plasma samples from 98 patients with BD and from 31 healthy controls using our in‐depth proteomics platform with a data‐independent acquisition mass spectrometer and antibody microarray. We performed bioinformatics analyses of the biologic processes and signaling pathways that were changed in the BD group and constructed a proteomics landscape of organ‐resolved BD pathogenesis. We then validated the biomarkers of disease severity and the vascular subset in an independent cohort of 108 BD patients and 29 healthy controls using an enzyme‐linked immunosorbent assay. Results The BD group had 220 differentially expressed proteins, which discriminated between BD patients (88.6%) and healthy controls (95.5%). The bioinformatics analyses revealed different biologic processes associated with BD pathogeneses, including complement activation, wound healing, angiogenesis, and leukocyte‐mediated immunity. Furthermore, the constructed proteomics landscape of organ‐resolved BD identified proteomics features of BD associated with different organs and protein targets that could be used for the development of therapeutic treatment. Hyaluronic binding protein 2, tenascin, and serpin A3 were validated as potential biomarkers for the clinical assessment of vascular BD and treatment targets. Conclusion Our results provide valuable insight into the pathogenesis of organ‐resolved BD in terms of proteomics characteristics and potential biomarkers for clinical assessment and potential therapies for vascular BD.

Coronary artery disease is a primary co-morbidity in metabolic diseases such as metabolic syndrome, diabetes and obesity. One contributing risk factor for coronary artery disease is low high-density lipoprotein-cholesterol (HDLc). Several factors influence steady-state HDLc levels, including diet, genetics and environment. Perhaps more important to coronary artery disease is factors that attribute to the dynamics of reverse cholesterol transport, storage, and excretion of excess cholesterol. HDLc biogenesis, clearance and innate ability to serve as a cholesterol acceptor and transporter all contribute to HDLc's function as a negative regulator of cardiovascular disease. With the recent failure of torcetrapid, focus is being placed on HDLc biology and its role in various metabolic diseases. Low HDLc levels are often associated with an increased state of background inflammation. Recently, several syndromes with clear pro-inflammatory components have been shown to be inversely correlated with low HDLc levels in the absence of obesity, diabetes and metabolic syndrome. Early studies with HDLc during the acute-phase response suggest that HDLc is substantially physically modified during acute infection and sepsis, and recent studies show that HDLc is physically modified by chronic pro-inflammatory disease. In this review, several of these connections are described and cytokine signalling related to HDLc is examined.
