Aim:The aim of this study was to estimate the salivary cortisol levels in patients with oral lichen planus (OLP) and recurrent aphthous stomatitis (RAS) and to correlate it with their psychological profile using the Hospital Anxiety and Depression (HAD) scale. Methods: The study population comprised of 60 patients -20 with OLP, 20 with RAS and 20 healthy controls. Salivary cortisol levels were assessed using the chemiluminescent immuno assay technique. Anxiety and depression levels were evaluated using the HAD scale. Results: The Kruskal Wallis mean ranks of salivary cortisol level in the OLP and RAS groups were 33.45 and 33.48 respectively and were higher than the mean rank of the control group with 24.58. But this difference was not statistically significant (P = 0.178). The mean ranks of anxiety score in the OLP and RAS groups were 36.48 and 39.50. The mean ranks of depression scores were 41.88 and 35.25 in the OLP and RAS groups respectively. The control group had a statistically significant lower mean ranks for anxiety and depression (P < 0.001). Anxiety and depression scores showed strong positive correlation in both patient groups. Conclusion: The results suggest that anxiety and depression play a role in the pathogenesis of OLP and RAS. The role of salivary cortisol as a biomarker of stress remains to be validated. Patients suffering from psychosomatic diseases such as OLP and RAS need a comprehensive and holistic treatment approach to manage effectively the psychosocial factors involved in the disease process. Oral liken planus (OLP)'lu bireylerde kortizol, anksiyete ve depresyon düzeylerinin değerlendirilmesi Amaç: Bu çalışmanın amacı; oral liken planus'lu bireylerde kortizol ve anksiyete/ depresyon düzeylerini değerlendirmek ve psikolojik faktörlerin oral liken planus (OLP)'un etyolojisindeki etkilerini saptamaktır. Yöntem: Marmara Üniversitesi Dişhekimliği Fakültesi Oral Diagnoz ve Radyoloji Anabilim Dalı Kliniği'ne başvuran, OLP klinik ön tanısı konmuş 30 birey hasta grubunu, sağlıklı 30 birey ise kontrol grubunu oluşturmuştur. OLP ön tanılı bireylerden kliniğimizde biyopsi materyali alınarak tanı histopatolojik olarak da desteklenmiştir. Hem çalışma hem de kontrol grubunun periferik kan kortizol düzeyleri değerlendirilmiş ve "Spielberger Durumluluk Kaygı Testi (DKT) -Süreklilik Kaygı Testi (SKT)" ile "Zung Depresyon Ölçeği (ZDP)" uygulanmıştır. Bulgular: Çalışma ve kontrol grubundaki hastaların kortizol, DKT ve ZDP değerleri ortalaması arasında anlamlı bir farklılık bulunmazken (p>0.05), çalışma grubunun SKT değerleri ortalaması kontrol grubundan istatistiksel olarak anlamlı düzeyde yüksektir (p<0.05). Sonuç: Oral Liken Planus'lu bireylerde süreklilik kaygı düzeyinin yüksek olması anksiyetenin bu hastalığın etyolojisinde önemli bir faktör olabileceğini ortaya koymaktadır. Method: 30 individuals pre-diagnosed with OLP who referred to Marmara University, Dentistry Faculty, Department of Oral Diagnosis and Radiology constitute the patient group while 30 healty individuals are control group. Pre-diagnosis of OLP of the patients was supported histopathologically with biopsies taken. Peripheral blood cortizol levels were evaluated and Spielberger State-Trait Anxiety Test with Zung depression Scale (ZDS) were administrated on both study and control group. Results: There was no statistical differences on the mean levels of Trait Anxiety Test and ZDS between the study and conrol group (p>0.05), while mean levels of State Anxiety Test of the study group was significantly higher than the mean levels of mean levels of State Anxiety Test of control group (p<0.05). Conclusion: Higher levels of state anxiety in individuals with oral lichen planus exhibits that anxiety is an important factor on the etiology of this disease.

Abstract: Because of the fact that the level of anxiety and salivary cortisol of OLP patients were high, our findings concluded that this disease is closely related with stress. Thus besides traditional treatment of OLP patients, our findings suggest that psychological support is also needed.
