eart transplantation has been used to treat dilated cardiomyopathy (DCM) in patients in whom medical therapy or a left ventricular assist device has failed. However, there are a limited number of hearts available for transplantation, and many patients die while awaiting transplantation. Recently, partial left ventriculectomy (PLV; the Batista procedure) 1-6 has received attention as an alternative or bridging treatment for severe cardiomyopathies. In this operation, the surgeon partially resects the left ventricle in an attempt to decrease left ventricular wall tension by reducing chamber volume. Therefore it is necessary to establish the indications for this procedure and to determine the prognosis following this surgery.Immunologic abnormalities, viral infections, and genetic predisposition are possible factors involved in the development of myocardial injury in DCM. 7 The presence of inflammatory cell infiltrates [8][9][10][11] and viral genomes [12][13][14][15] 16,17 It is therefore quite important to determine the role of inflammatory cell infiltrates or active myocarditis in the development of DCM.The histopathologic features of myocardial injury and the severity of cardiomyopathy are considered to be factors determining the prognosis following PLV. We previously reported that severe inflammatory cell infiltration might serve as a predictor of poor outcome, 18,19 but we could not determine a significant correlation between any histopathologic findings and the early prognosis of patients with DCM following PLV, because the number of patients was too small.The purpose of present study was to determine the involvement of active myocarditis in DCM using histopathologic, immunohistochemical and virologic studies of myocardial specimens obtained by PLV. Arrhythmogenic Right Ventricular Cardiomyopathy Causing Sudden Cardiac Death in Boxer Dogs

Background-Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a primary familial heart muscle disease associated with substantial cardiovascular morbidity and risk of sudden death. Efforts to discern relevant pathophysiological mechanisms have been impaired by lack of a suitable animal model. Methods and Results-ARVC was diagnosed in 23 boxer dogs (12 male; 9.1Ϯ2.3 years old). Clinical events alone or in combination included sudden death (nϭ9; 39%), ventricular arrhythmias of suspected right ventricular (RV) origin (nϭ19; 83%), syncope (nϭ12, 52%), and heart failure (nϭ3; 13%). Right ventricular enlargement or aneurysms occurred in 10 (43%). Striking histopathological abnormalities were present in each boxer dog but not in controls, including severe RV myocyte loss with replacement by fatty (nϭ15, 65%) or fibrofatty (nϭ8, 35%) tissue. Focal fibrofatty lesions were also present in both atria (nϭ8) and the left ventricle (LV) (nϭ11). Fatty replacement occupied substantially greater RV wall area in ARVC dogs than controls (40.4Ϯ18.8% versus 13.8Ϯ3.4%, respectively) (PϽ0.001); residual myocardium was correspondingly reduced (56.6Ϯ19.2% versus 84.8Ϯ3.8% in controls) (PϽ0.001). MRI demonstrated bright anterolateral and/or infundibular RV myocardial signals, confirmed as fat by histopathology. Myocarditis appeared in the RV (nϭ14, 61%) and LV (nϭ16, 70%) and in each dog with sudden death, but not in controls. Familial transmission was evident in 10 of the 23. In light of advances in genomic mapping of the domestic dog, 8 a spontaneous canine model of ARVC and sudden death would provide a unique opportunity to study this disease genome and contribute valuable insights into its pathogenesis. Therefore, the purpose of the present study was to define the clinical and pathological features of a naturally occurring myocardial disease in boxer dogs and assess its suitability as an animal model of ARVC. Immunosuppressive Treatment for Inflammatory Cardiomyopathy: Meta-Analysis of Randomized Controlled Trials

SUMMARYThe use of immunosuppressive therapy for inflammatory cardiomyopathy is controversial.The aim of this review is to summarize the current empirical evidence for immunosuppressive treatment in inflammatory cardiomyopathy.We conducted a meta-analysis of all randomized controlled trials (RCTs) that compared immunosuppressive therapy with either placebo or conventional treatment in patients with inflammatory cardiomyopathy. The pooled outcomes were all-cause death and heart transplantation for inflammatory cardiomyopathy, left ventricular ejection fraction (LVEF), and left ventricular end diastolic dimension (LVEDD).Five trials involving 316 patients were included. Overall, immunosuppressive therapy was not superior to placebo and conventional therapy in the pooled outcome of all-cause death and heart transplantation (odds ratio [OR] 1.03, 95% confidence intervals [CI] 0.58 to 1.80) in the long-term. The pooled data showed that there might be a short-term beneficial effect on LVEF improvement (5.06%, 95% CI -0.07% to 10.18%) in patients receiving immunosuppressive therapy, but no beneficial effect on LVEDD either in the short-term (-0.87 mm; 95% CI, -8.29 to 6.55 in adult patients) or long-term (-0.52 mm, 95% CI -3.64 to 2.60 in adult patients) was observed.There is no evidence to suggest that immunosuppressive therapy has an effect on improving the survival of patients with inflammatory cardiomyopathy. Current therapy in inflammatory cardiomyopathy seems to be limited to supportive measures or transplantation. (Int Heart J 2005; 46: 113-122)
