The C677T polymorphism of the methylenetetrahydrofolate reductase (MTHFR) gene was implicated to be associated with thrombophilia due to its role in catalyzing the formation of 5-methylenetetrahydrofolate, a co-substrate for the conversion of homocysteine to methionine. Several case-control studies were investigated MTHFR C677T polymorphism as risk for recurrent pregnancy loss (RPL). These studies rendered contradictory results, some indicating that the polymorphism is associated with the risk of RPL whereas others concluded there is no association. To shed light on these inconclusive findings, a meta-analysis of all available studies published from Asian population relating the C677T polymorphism to the risk of RPL was conducted. The following electronic databases were searched without language restrictions: PubMed, Google Scholars, Elsevier and Springer Link up to December, 2015. Meta-analysis was performed using MetaAnalyst and Mix version 1.7. Meta-analysis results suggested that MTHFR C677T polymorphism contributed to the increased RPL risk in Asian population using all five genetic models (for T vs. C: OR 1.35, 95 % CI 1.09-1.68, p = 0.009; for TT ? CT vs. CC: OR 1.44, 95 % CI 1.14-1.82, p = 0.006; for CT vs. CC: OR 1.39, 95 % CI 1.07-1.8, p = 0.01; for TT vs. CC: OR 1.79, 95 % CI 1.23.2.6, p = 0.007; for TT vs. CT ? CC: OR 1.61, 95 % CI 1.02-2.56, p = 0.04). In conclusion, this meta-analysis demonstrates a strong association between the MTHFR C677T variant and RPL in Asian population and raising the importance of the use of folate in its treatment and prevention.

Background: Anticardiolipin antibodies (ACA) are found with increased prevalence in women with unexplained recurrent pregnancy loss but their impact on future pregnancy is not clearly known. Therefore, present study objective was to compare the prevalence of ACA in patients with recurrent pregnancy loss and normal healthy pregnant women.Methods: This prospective observational study from 100 pregnant women attending the outpatient department of Obstetrics and Gynaecology were studied. Study included 50 pregnant women with a history of three or more consecutive unexplained pregnancy loss as a study group and 50 pregnant women with two or more live children with no history of any previous pregnancy loss as a control group. Exclusion criteria included cases with any uterine anomaly, uterine structural abnormalities, induced abortions, infections, systemic disease, Rh-ve pregnancy, coagulation disorders and personal or family history of thrombosis.Results: Overall, 9 patients tested positive for ACA, of which one (2%) patient was from the control group and remaining 8 (16%) were from study group. Overall, 42 (84%) patients had negative test for ACA in the study group and 49 (98%) patients in the control group. The live birth rate among study group was only 34 (68%) whereas all 50 (100%) pregnant women in the control group.Conclusions: The findings described here in demonstrate that, ACA -positive patients are also the primary predictor of adverse pregnancy outcome and obstetric complications. Therefore, the role of early detection of ACA in all pregnant women is very important to prevent foetal outcomes and obstetric complications.

Aims:The aim of our study was to observe the benefit of screening for thyroid function amongst women with recurrent pregnancy loss and effect of treatment for thyroid disorder on pregnancy outcome. Methods:This was a longitudinal study conducted from June 2012 to December 2013. One hundred and three patients with recurrent pregnancy loss without features of thyroid disorder were included. They were investigated for thyroid stimulating hormone (TSH), free tri iodothyronin (FT 3 ), free thyroxine (FT 4 ) levels and for auto-antibodies against thyroperoxidase (anti TPO). The patients with abnormal TSH levels were treated with thyroxine depending on the level of TSH.Results: TSH, FT 3 , FT 4 and anti TPO levels were measured. Amongst 103 ladies, thirty-eight (36.89%) had high levels of TSH. Thirty-five (33.98%) of them underwent test for anti TPO, of which two (5.71%) had autoimmune thyroiditis. Nine (8.73%) out of 103 had high FT 4 levels. Ladies with diagnosis of hypothyroidism underwent treatment and 17 (44.73%) out of 38 had conceived. Conclusions:Of 103 women with recurrent pregnancy loss, abnormal thyroid function was detected in 47. Seventeen (44.73%) of them with increased TSH level conceived after treatment with thyroxine, which gives a positive message that hypothyroidism is a treatable cause for recurrent pregnancy loss.
