The Erector Spinae Plane Block

Abstract: Thoracic neuropathic pain is a debilitating condition that is often poorly responsive to oral and topical pharmacotherapy. The benefit of interventional nerve block procedures is unclear due to a paucity of evidence and the invasiveness of the described techniques. In this report, we describe a novel interfascial plane block, the erector spinae plane (ESP) block, and its successful application in 2 cases of severe neuropathic pain (the first resulting from metastatic disease of the ribs, and the second from ma… Show more

“…This case documents a new alternative for inguinal hernia repair in newborns and infants. The effectiveness of the ESP block is explained by LA spread along the erector spinae muscle and toward the paravertebral space providing somatic and visceral pain control …”

“…An erector spinae plane block (ESPB) is a tissue plane block that was first described in adults in 2016 . Since its introduction, the ESPB has been used in adults to provide analgesia for a wide range of surgery including open and laparoscopic abdominal and cardiothoracic surgeries, hip arthroplasties, prostatectomies, upper extremity amputations, and cervical spine fusions .…”

In addition to acute intraoperative and postoperative analgesia, ESPBs have also been described for chronic and non‐surgical pain, as well as a regional technique to facilitate weaning from the ventilator in the intensive care unit …”

“…An injectate mixture of bupivacaine 0.25% 6 to 10 mL and methylprednisolone 40 to 80 mg was administered to the target area. The erector spinae plane blocks were performed deep to the erector spinae muscle, which may lead to a greater spread of the medication in comparison to a block performed superficially to the erector spinae muscle . Additionally, patients with localized midline thoracic back pain were injected with the needle directed from lateral to medial to the medial tip of the transverse process, while patients with associated lateral radiation of their pain had the needle directed from medial to lateral to the lateral border of the transverse process.…”
