Purpose:To investigate the efficacy of dorzolamide 1% (bid or tid) or brinzolamide 1% bid on 24-hour intraocular pressure (IOP) control as well as patients' preference for either drug when added in combination with latanoprost against glaucoma (IOP, 18 mmHg). Methods: In this randomized crossover study patients were assigned to receive latanoprost plus either dorzolamide or brinzolamide for four weeks. Thereafter, patients underwent 24-hour IOP monitoring while continuing to receive dorzolamide (for two successive days/nights: at first bid then tid) or brinzolamide bid (once overnight). They were then switched over to receive the other test medication for a further four weeks and subsequently reexamined for 24-hour IOP. A questionnaire survey on treatment satisfaction was performed. Results: In 20 patients dorzolamide bid or tid or brinzolamide bid exerted significant (p  0.001) reductions of IOP from baseline at all time-points over 24 hours; no difference was detected among the treatment regimens. Significantly (p  0.05) more patients preferred dorzolamide (n = 9) over brinzolamide (n = 2), whereas nine patients gave a neutral answer. Conclusion: Dorzolamide bid or tid and brinzolamide bid when combined with latanoprost therapy elicited significant IOP reduction for 24 hours. It is rational to consider patients' preference of therapeutic regimen especially long-term users such as those with glaucoma.

Brinzolamide 1%/timolol 0.5% is a new fixed-combination for the treatment of open-angle glaucoma or ocular hypertension. Brinzolamide/timolol has a favorable safety profile, with an incidence of ocular burning and stinging <5%. Published data show that brinzolamide 1%/timolol 0.5% and dorzolamide 2%/timolol 0.5% have similar efficacies for lowering intraocular pressure (IOP). There is some evidence that brinzolamide/timolol may be more comfortable. Although patients receiving brinzolamide/timolol may experience more blurred vision on instillation, some data show a preference for brinzolamide/timolol over dorzolamide/timolol. Although available data to assess the role of brinzolamide/timolol in daily clinical practice are still limited, these first results suggest the agent to be a reasonable alternative for patients who do not reach target IOP with monotherapy.

RESUMO -O aumento da pressão intra-ocular (PIO) é o fator de risco primário para o desenvolvimento da neuropatia óptica glaucomatosa canina. O cloridrato de dorzolamida a 2% reduz a PIO pela diminuição da produção do humor aquoso. O objetivo do trabalho foi avaliar a ação do cloridrato de dorzolamida em solução a 2% na PIO de cães normais e verificar possíveis alterações no olho sem tratamento. A mensuração da PIO foi realizada em ambos os olhos, 1 hora antes e 1, 2, 4, 6 e 8 horas após a instilação de uma gota do colírio hipotensor no olho esquerdo, utilizando-se tonômetro de aplanação. O olho não tratado apresentou redução significativa da PIO, sendo que a redução foi de 13,8% e 22,4% no olho tratado. A administração tópica do cloridrato de dorzolamida 2% resultou na redução significativa da PIO de cães clinicamente normais. Dorzolamide is used for the treatment of glaucoma. The aim of this work was to evaluate the efficacy of 2% solution dorzolamide on IOP of healthy dogs. Twenty adult dogs received topical treatment in the left eye with 2% dorzolamide. IOP measurements were carried out using aplanation tonometry (Tono-penâ). Measurements were performed 1 hour before instillation of a single drop of the dorzolamide in the left eye, and at 1, 2, 4, 6 and 8 hours after treatment. In the no treated eye, IOP decreased over time, with a maximum decrease from baseline of 13,8%. In the treated eye, there was a reduction of 22,4%. It was concluded that 2% dorzolamide is effective to reduce IOP in healthy dogs.Key-words: Dog, intraocular pressure, glaucoma, dorzolamide. IntroduçãoO glaucoma é uma neuropatia óptica progressiva, geralmente associada ao aumento da pressão intra-ocular (PIO) (SCHWARTZ, 2001). Reveste-se de singular importância, como afecção de difícil conduta terapêutica e potencial causador de cegueira em cães, afetando 0,5 % dos cães (GELATT, 2003). O tratamento clínico do glaucoma canino recebe grande destaque, pois os procedimentos cirúrgicos têm somente 30 a 50% de sucesso na redução da PIO (GELATT e GELATT, 2001).A dorzolamida atua como inibidor da anidrase carbônica, sendo esta fundamental no processo de produção do humor aquoso (TSUKAMOTO e LARSSON, 2004). Esse fármaco foi introduzido na oftalmologia e rapidamente utilizado na terapia de pacientes glaucomatosos caninos (SLATTER, 2005). Segundo CARWSE et al.
