Нистагм – важнейший объективный симптом, свидетельствующий о вестибулярной дисфункции (врожденная или приобретенная в процессе различных заболеваний). Доброкачественное пароксизмальное позиционное головокружение (ДППГ), по нашим данным, является наиболее распространенным заболеванием периферического отдела вестибулярного аппарата и характеризуется преходящим ощущением вращения, которое сопровождается характерным нистагмом. Одной из форм спонтанного нистагма является врожденный нистагм, связанный с врожденными дефектами развития системы управления микродвижениями глаз. В рассмотренном клиническом случае (пациент – 35-летний мужчина с жалобами на периодическое позиционное несистемное головокружение) представлены два типа нистагменных реакций: спонтанный и провокационный нистагм. При этом выявленный спонтанный нистагм и его особенности (выявление фиксации взора на объекте, исчезновение при отсутствии фиксации взора на объекте, инверсия ОКН при оптокинетической стимуляции, выявление при подавлении вестибулоокулярного рефлекса фиксацией взора) соответствуют диагностическим критериям врожденного нистагма. Выявление у пациента провокационного горизонтального нистагма без латентного периода длительностью до 50 сек. при проведении пробы Дикса – Холлпайка и пробы МакКлюра – Пагнини без фиксации взора с отсутствием П-образного спонтанного нистагма является диагностическим критерием купулолитиаза горизонтального полукружного канала слева. При установлении ДППГ при наличии у пациента ВН лечение направлено на репозицию отолитов и включает в себя репозиционные маневры и вестибулярную гимнастику. Nystagmus is the most important objective symptom indicating vestibular dysfunction (congenital or acquired in the course of various diseases). Benign paroxysmal positional vertigo (BPPV) according to our data is the most common disease of the peripheral vestibular apparatus and is characterized by a transient sensation of rotation, which is accompanied by a characteristic nystagmus. One of the forms of spontaneous nystagmus is congenital nystagmus (CN) associated with congenital defects in the development of the micro-movement control system of the eyes. The patient is a 35-year-old man with complaints of periodic positional non-systemic dizziness. In the presented clinical case, two types of post-inflammatory reactions are presented: spontaneous and provocative nystagmus. At the same time, the revealed spontaneous nystagmus and its features (detection of fixation of the gaze on the object, disappearance in the absence of fixation of the gaze on the object, inversion of the optokinetic nystagmus (OKN) during optokinetic stimulation, detection when the vestibulocular reflex is suppressed by fixation of the gaze) correspond to the diagnostic criteria of congenital nystagmus. Detection of provocative horizontal nystagmus in a patient without a latent period lasting up to 50 seconds when conducting the Dix – Hallpike test and the McClure – Pagnini test without fixing the gaze with the absence of a U-shaped spontaneous nystagmus, it is a diagnostic criterion for cupulolithiasis of the horizontal semicircular canal on the left. When BPPV is established in the presence of CN in the patient, treatment is aimed at the reposition of otoliths and includes repositional maneuvers and vestibular gymnastics.

Para los médicos de atención primaria, el paciente con sintomatología compatible con síndromes vestibulares constituye un desafío, debido a su amplia variedad de presentaciones y la dificultad en la diferenciación de las diversas patologías que ocasionan los síntomas. La necesidad de tener conceptos claros y actualizados sobre los mecanismos patológicos del vértigo surge en el momento de evaluar al paciente, para esto es necesario realizar un adecuado interrogatorio y un examen físico neurotológico completo que permita dilucidar un abordaje diagnóstico certero. El objetivo del presente artículo es presentar una revisión de la literatura actual, de las diferentes etiologías desencadenantes del vértigo periférico y a partir de sus características clínicas desarrollar un algoritmo diagnóstico que permita al profesional médico entender el mecanismo patológico del vértigo y proporcionar el tratamiento adecuado.

The objective of the present review of the literature is the analysis of the currently available data concerning etiology and pathogenesis of benign paroxysmal positional vertigo (BPPV). The special emphasis is placed on the modern hypotheses of BPPV formation that collectively account for not more than 15% of all known cases of this condition. The best explored are the following causes of benign paroxysmal positional vertigo: vestibular neuronitis, head injuries, and disorders in the middle ear. During the recent years, much attention has been given to the role of disturbances of calcium metabolism and osteoporosis in etiology of benign paroxysmal positional vertigo. It is supposed that pathogenesis of vertiginous attacks can be explained in terms of the canalolithiasis and cupulolithiasis theories.
