Aim:The aim of this study is to investigate the effect of pentoxifylline (PTX) and milrinone on the stasis zone in rats for which an experimental burn model was established. Method: In this study with an experimental design, PTX and milrinone -5-phosphodiesterase inhibitors -were administered orally and intraperitoneally to rats for which an experimental burn model was created. The effects of these pharmacological agents on the stasis zone and comparison of these effects with each other were investigated. In the study, 56 4-monthold female rats of Wistar albino type with a weight of 200 ± 10% g were used. The rats were divided into seven groups. The burn model defined by Regas and Erlich was applied to 48 rats under anesthesia. Groups 1, 2, and 3 were defined as the sham group, intraperitoneal control group, and oral control group, respectively. Drugs were administered to groups for 10 days as follows: PTX intraperitoneally daily at a dose of 50 mg/kg to Group 4, PTX orally daily at a dose of 50 mg/kg to Group 5, milrinone intraperitoneally daily at a dose of 1 mg/kg to Group 6, and milrinone orally daily at a dose of 1 mg/kg to Group 7. All rats were sacrificed on the 10 th day of the experiment. Serum samples for the biochemical levels of malondialdehyde (MDA), glutathione peroxidase (GPx), superoxide dismutase (SOD), and catalase were taken as well as tissue samples for the histopathological study of these. Results: Pathologically, edema, hyperemia, epithelial degeneration, necrosis, inflammatory filtration, and fibrosis measurements were taken. Compared to the control group, the tissue damage score was lower in all treated groups. MDA levels were lower in the intraperitoneal and oral PTX administered group than in the control group, while SOD, catalase, and GPx levels were higher. MDA levels were lower in the intraperitoneal and oral milrinoneadministered group than in the control group, and SOD, catalase, and GPx levels were higher. In addition, it was found that MDA levels were lower and SOD, catalase, and GPx levels were higher in the intraperitoneal PTX-administered group than in the intraperitoneal milrinone-administered group. It was found that MDA levels were lower and SOD, catalase, and GPx levels were higher in the oral PTX-administered group than in the oral milrinone-administered group. Conclusion: As a result of our study, it was shown that PTX and milrinone reduce oxidative stress and have positive effects, improving wound healing in the burn model established for the rats.

Εισαγωγή: Πραγματοποίηθηκε έλεγχος της υπόθεσης ότι η τοπική εφαρμογή αγγειοδραστικών ουσιών, διατηρεί την αιματική ροη σε θερμική κάκωση στο δέρμα, περιορίζοντας την εξέλιξη της ιστικής νέκρωσης στην αρχική φάση του εγκαύματος. Σκοπός: Ο σκοπός αυτής της τυχαιοποιημένης πειραματικής μελέτης, ήταν να ελέγξει πιθανή βελτίωση της αιματικής ροής μετά από την τοπική εφαρμογή αγγειοδιασταλτικών ουσιών.Υλικό και Μέθοδος: Χρησιμοποιήθηκαν 55 επίμυες οι οποίοι χωρίστηκαν σε 7 ομάδες, ώστε να μελετηθεί η δράση της διαδερμικής εφαρμογής αλοιφής τρινιτρογλυκερίνης περιεκτικότητας 0,4% (Rectogesic®) (Ομάδα Α, n=10), κρέμας πριλοκαΐνης/λιδοκαΐνης περιεκτικότητας 5% (Emla®) (Ομάδα B, n=10) σε σύγκριση με τη μη εφαρμογή ουσίας (Ομάδα Γ, n=10) ή την εφαρμογή σκευάσματος placebo (Ομάδα Δ, n=10) και η δράση του διαλύματος λιδοκαΐνης 2% (Xylocaine® 2%) (Ομάδα Ε, n=5) με τοπική έγχυση, σε σύγκριση με τη μη έγχυση ή την έγχυση διαλύματος placebo (Ομάδα ΣΤ, n=5), στην αιμάτωση βαθέων εγκαυμάτων μερικού πάχους. Όλες οι ουσίες ελέγχθηκαν κατ’ αντιστοιχία και σε σύγκριση με μάρτυρες χωρίς έγκαυμα (Ομάδα Ζ, n=5). Αποτελέσματα: Κατά τη μέτρηση της αιματικής ροής σε ml/min/100gr με ροόμετρο Laser Doppler στα 15, 30, 45, 60, 120 και 180 λεπτά, αποδείχθηκε ότι διακυμάνθηκε από 25,3750 (SD 14,05502) έως 31,9020 (SD 24,92093) στην Oμάδα Α, από 20,2500 (SD 8,39248) έως 24,4917 (SD 14,25861) στην Oμάδα Β, από 11,6000 (SD 4,84271) έως 14,5833 (SD 7,62478) στην Oμάδα Γ, από 11,2877 (SD 5,36349) έως 13,1924 ( SD 5,98779) στην Oμάδα Δ, από 5,3041 (SD 2,46069) έως 7,2485 (SD 4,59262) στην Oμάδα Ε και από 8,4667 (SD 3,85393) έως 10,7 (SD 5,93296) στην Oμάδα ΣΤ. Η φυσιολογική ροή του αίματος διατηρήθηκε σε ποσοστό 45,2-81,0% (64,36%) στην Oμάδα Α, 50,8-73,2% (62,31%) στην Oμάδα Β, 44,4-57% (50,66%) στην Oμάδα Γ, 37,1-53,4% (43,63%) στην Oμάδα Δ , 39,6-65,7% (49,05%) στην Oμάδα Ε και 25,2-60% (40,05%) στην Oμάδα ΣΤ. Η σύγκριση με τους μάρτυρες χωρίς έγκαυμα έδειξε βελτίωση της ροής στην Oμάδα Α κατά τη μέτρηση στα 15 λεπτά (p=0.008) και στην Oμάδα Β στα 120 (p=0,020) και στα 180 (p=0,011) λεπτά, ενώ έδειξε μειωμένη ροή στην Oμάδα Δ (p<0,001), Ε (p<0.05) και ΣΤ (p<0.05). Η εφαρμογή αλοιφής Rectogesic®, απέδειξε βελτίωση ροής σε όλες τις μετρήσεις σε σύγκριση με την εφαρμογή σκευάσματος placebo (p<0,05) ή τη μη εφαρμογή ουσίας (p<0,05). Η εφαρμογή κρέμας Emla® απέδειξε βελτίωση ροής σε σύγκριση με την εφαρμογή σκευάσματος placebo (p<0,05) κατά τις μετρήσεις στα 15, 60, 120 και 180 λεπτά και σε σύγκριση με τη μη εφαρμογή ουσίας κατά τις μετρήσεις στα 15, 30, 120 και 180 λεπτά. H διαφορά από την εφαρμογή των δύο ουσιών, δεν ήταν στατιστικά σημαντική. Τα ευρήματα αυτά υποστηρίζουν την αρχική υπόθεση. Η τοπική έγχυση Xylocaine® 2% μείωσε την ροή σε σύγκριση με τη μη εφαρμογή (p<0.05) στις μετρήσεις στα 15, 45, 60, 120 και 180 λεπτά και δεν παρουσίασε στατιστικά σημαντική διαφορά από την έγχυση διαλύματος placebo. Το εύρημα αυτό δεν υποστηρίζει την αρχική υπόθεση. Συμπέρασμα: H διαδερμική εφαρμογή αλοιφής Rectogesic® και κρέμας Emla® παρουσιάζει κλινική αξία, αφού προσφέρουν παρόμοια βελτίωση της αιματικής ροής. Το συμπέρασμα αυτό δεν ισχύει στην περίπτωση τοπικής έγχυσης Xylocaine® 2%.

Experimental burn models are always needed, being essential for burn research. In the present study, we describe a standard Fischer F-344 rat burn model, in which burns are produced with a heated brass bar. The animals were anesthetized with ketamine (90 mg/kg). After the burns, they were resuscitated with saline. Burns ranging from 26% to 30% of total body surface area resulted in a 62.5 % mortality rate after 25 days. This model is an additional option for those interested in burn research.
