The Effects of Nd:YAG Laser Posterior Capsulotomy on Macular Thickness, Intraocular Pressure, and Visual Acuity

Abstract: Increased IOP and macular thickness are inevitable after Nd:YAG laser capsulotomy, but the severity and duration are less when a total energy level less than 80 mJ is used.

“…Similarly, in a study performed by reviewing the applied force and complication, the authors (4) found that CMT change was significant in cases where >80 mj was applied, but insignificant when a force <80 mj was applied. Here if we consider that the necessary dose is applied to open the capsule, this means that a higher dose results in a more rigid and thick capsule and the release of more capsular degradation products.…”

“…Moreover, retinal complications that may emerge later affect the choroid tissue. Various studies associated with anterior chamber and retinal changes in patients who have undergone YAG capsulotomy have been published (4)(5)(6) . However, to the best of our knowledge, there are no publications regarding choroi dal change.…”

“…(16) Ari et al who examined 30 patients according to the energy levels, they found significant increase in CMT in high energy group. (17) Another study from Turkey, Yılmaz et al studied about long term changes in macular thickness, they did not find significant change in CMT. (18) Casas et al found insignificant change in CMT at 3 months after treatment.…”

“…Capsulotomy is considered a relatively safe procedure, but it still has several complications. These include a sudden increase in intraocular pressure (IOP), long-term IOP elevation [10111213], iris hemorrhage [14], cystoid macular edema [121516], posterior vitreous detachment [17], retinal breaks/detachment [181920], and aqueous misdirection syndrome [2122]. Neovascular glaucoma (NVG) rarely occurs as a complication of capsulotomy [23].…”
