Objective. The present study assessed the chemical elements in two novel calcium silicate-containing root canal sealers, BioRoot RCS and Well-Root ST, compared to a calcium silicate-containing root canal sealer that has been on the market for several years, MTA Fillapex, and epoxy resin-based sealer AHPlus. Material and Methods. The sealers were mixed and manipulated according to the manufacturers' instructions. Twelve cylindrical molds (inner diameter 4 mm; height 3 mm) were placed on a glass petri dish and packed with the materials. The dish was transferred to an incubator. After 72 h the molds were examined by scanning electron microscopy and energy dispersive X-ray microanalysis. Results. BioRoot RCS and Well-Root ST had high peaks of calcium, zirconium, oxygen, carbon, silicon, and chlorine. Well-Root ST also had sodium, magnesium, aluminum, and titanium peaks. MTA Fillapex and AHPlus had carbon, oxygen, calcium, titanium, and bismuth peaks. A silicon peak was also observed for MTA Fillapex, and zirconium and tungsten peaks for AHPlus. Conclusion. BioRoot RSC had the highest degree of purity. The clinical implication of metals contained in the other sealers needs to be investigated.

Aim To evaluate the in vitro cytotoxicity and cytokine release of three fresh root canal sealers and to determine the type of cell death they induce. Methodology The sealers tested were Sealer 26 (S26), AH Plus (AHP), and Endosequence BC Sealer (END). Fresh sealers were cultivated in contact with monocytes and polymorphonuclears (PMNs) obtained from the peripheral blood of humans. Cell viability, apoptosis and necrosis were analysed at 4 h (PMNs) or 24 h (monocytes) using Annexin‐V and propidium iodide in a cytometer. The supernatants were used to quantify Interleukin (IL)‐4, IL‐6, IL‐10, IL‐12 and tumour necrosis factor‐α (TNF‐α) in monocytes and IL‐8 in PMNs by ELISA. One‐way ANOVA and the Tukey post‐test were used to compare data for cytotoxicity, and the multiple T‐test was used to determine the differences between sealers in the release of cytokines that were statistically significant. Results After 4 h of treatment, S26 was associated with greater cell viability than the other sealers (P < 0.05) in the PMN culture and had similar values of necrosis as END (P > 0.05). After 24 h of treatment, AHP and END had greater monocyte cell viability than S26 (P < 0.05), which had more necrosis (P < 0.05). END had the lowest levels of IL‐12 compared to the other sealers (P < 0.05) and higher levels of IL‐6 compared to S26 (P < 0.05). The tested sealers did not differ in the release of IL‐8, IL‐10, TNF‐α and IL‐4 (P > 0.05). Conclusions The effect of toxic agents released varied depending on the cell type studied. The composition of the sealers appeared to alter the form of self‐regulation in the production of these cytokines by cells.

The principal function of root filling materials is to prevent invasion of bacteria and infection of the root canal system after completed endodontic treatment. This is accomplished by the formation of a tight, permanent seal with the surrounding tooth structure, leaving no space for invasion of or colonization by bacteria. The materials can also have a direct antibacterial effect and kill the microbes when in contact with them. Materials applied onto larger areas of connective tissue, i.e. for retrograde root fillings or perforation repair, should also allow or stimulate complete repair or regeneration. Ideally, the selection of materials for various applications in endodontics should be based on clinical, scientifically valid data. However, most current endodontic practices and choice of materials are based on a convoluted history where data of varying relevance and validity is mixed with experts’ stated opinions and manufacturers’ advertising. In this review, the chemical, physical, and biological properties of different root filling materials are discussed together with their handling characteristics and recommended use in the clinic. Emphasis is placed on evidence generated from clinical practice and research.
