Background We report a high-risk case of a coronavirus disease 19 (COVID-19)-positive patient with comorbidities including diabetes mellitus (DM), hypertension (HTN), hypothyroidism and chronic kidney disease (CKD), treated successfully using an integrative therapy plan based on Ayurveda and Yoga, along with government-mandated compulsory modern western medicine (MWM) treatment. Recently, some evidence has been emerging on the use of Ayurveda for treatment of COVID-19. The classical texts of Ayurvedic medicine such as Charaka Samhita and Sushruta Samhita contain descriptions of pandemics of similar proportions and describe them as Janapadoddhvansa, meaning the destruction of communities, along with their causes and treatment. Case presentation The case reported herein is a 55-year-old man from Delhi, India, with confirmed (tested) COVID-19, who first took MWM for 7 days before seeking integrative therapy. The patient has comorbidities including DM, HTN, hypothyroidism and CKD and had developed symptoms including fever (which was resolved by the time integrative therapy was started), sore throat, dry cough, body aches, weakness, bad taste and smell, and heaviness in the abdomen. Based on the patient’s symptoms and comorbidities, a treatment plan including Ayurvedic medicines, Yoga protocol, dietary recommendations and lifestyle modifications was prescribed by a registered Ayurveda doctor and a Yoga consultant. The patient started experiencing improvement in all the symptoms within 2 days after starting the treatment; he reported approximately $$75\%$$ 75 % relief from the symptoms after 5 days, and almost complete relief within 9 days. Also, the blood sugar levels (both fasting blood sugar [FBS] and postprandial blood sugar [PPBS]) exhibited significant improvement after 5 days, and decreased to within the normal range within 12 days. Besides relief in symptoms, the patient’s real-time reverse transcription polymerase chain reaction (RT-PCR) test done on the 19th day returned negative results. Conclusions Integrative therapy was found to be effective in mitigating the symptoms of COVID-19 in this patient with multiple comorbidities. Moreover, a significant improvement in blood sugar levels (not under control with modern medicine) was also achieved. Integrative therapy based on the classical texts of Ayurveda and Yoga may offer a promising and scalable treatment option for COVID-19 patients. A case series or a suitably designed randomized controlled trial is needed to assess its efficacy.

Background: People who have underlying medical conditions and those over 60 years old have a higher risk of developing coronavirus disease complications (COVID-19) with more mortality rate. Breathing exercises is good for clearing the mucus from lungs and improves chest expansion while circulatory exercises enhance cardiopulmonary/vascular systems. Aim: This study aims to suggest exercise program for prevention, decrease morbidity and mortality in COVID-19 patients and patients with other respiratory co-morbidities. Conclusion: Regular simple exercises include respiratory and circulatory exercises may have a great effect on improving lung function and immunity in addition to relieving psychological stress thus can help in treatment of high risk people from COVID-19.

Background Providing integrated patient-centred care based on the patient’s needs has emerged as the first pillar of tuberculosis prevention and control strategies. However, a reliable and valid instrument is still lacking. Hence, the aim of this study was to develop and psychometrically test a new scale for assessing the supportive care needs of patients with pulmonary tuberculosis. Methods A methodological study involving item generation and psychometric evaluation was used. Based on the Supportive Care Needs Framework, the item-pool was drafted from a systematic review of the literature, expert consultations and feedback from a pilot study. A convenience sample of 518 patients was recruited from four hospitals in Shaan’xi Province from September 10, 2019, to January 20, 2020. Item reduction and scale validation were assessed for content validity, construct validity, Cronbach’s alpha coefficient, half-split reliability, and test-retest reliability. Results The final scale encompasses 25 items in five domains: practical, psycho-emotional, social, information and physical. The item level content validity index ranged from 0.80 to 1.00, and the scale level content validity was 0.925. Five factors that explained 80% of the total variance were also identified in exploratory factor analysis and confirmed in confirmatory factor analysis. The CFA model fit indices were x2/df = 2.229 (p < 0.001), GFI = 0.853, AGFI = 0.820, RMSEA = 0.069. All items had acceptable convergent and discriminant validity. Cronbach’s α coefficient, the split-half reliability coefficient and the test-retest reliability coefficient were 0.884, 0.883 and 0.854, respectively. Conclusions This newly developed scale demonstrated acceptable reliability and validity, and it could be used to assess the supportive care needs of pulmonary tuberculosis patients in clinical settings.
