He works as consultant rheumatologist in a large department of rheumatology. His department has taken part in clinical trials of etanercept, adalimumab, rituximab and tocilizumab. The department receives funding for nurses from Schering-Plough Ltd to administer infliximab to patients currently being treated with this drug. Currently he is the chief investigator of a clinical trial comparing adalimumab versus etanercept (ISRCTN95861172). Сеченова, Москва, Российская Федерация Лечение пациентов с ювенильным идиопатическим артритом (ЮИА) остается одной из наиболее сложных и актуальных проблем ревматологии. Цель исследования: оценить эффективность и безопасность длительной терапии адалимума-бом в сочетании с иммунодепрессантами у пациентов с ЮИА без системных проявлений. Методы: проведено моно-центровое наблюдательное сравнительное исследование. Изучали результаты лечения пациентов с ЮИА, получавших адалимумаб в сочетании с иммунодепрессантами (n = 215) и метотрексат (n = 200). Оценку эффективности терапии проводили с использованием педиатрических критериев Американской коллегии ревматологов (АКР педи) ОБОСНОВАНИЕЮвенильный идиопатический артрит (ЮИА) -системное хроническое заболевание, развивающее-ся у детей в возрасте до 16 лет, характеризующее-ся преимущественным поражением суставов, а также патологией других органов и тканей с формировани-ем полиорганной недостаточности различной степени выраженности [1].Лечение ювенильного артрита остается одной из наиболее сложных и актуальных проблем ревма-тологии [2]. В лечении этого заболевания используют широкий спектр противоревматических лекарственных средств: нестероидные противовоспалительные пре-параты (НПВП), глюкокортикоиды (для перорального, внутривенного и внутрисуставного введения), хиноли-новые производные, Д-пеницилламин, препараты золо-та, сульфасалазин, лефлуномид; иммунодепрессанты из группы цитостатиков (циклофосфамид, азатиоприн, хлорамбуцил), антиметаболитов (метотрексат); селек-тивные иммунодепрессанты (циклоспорин) [3,4]. Однако у многих пациентов по-прежнему не удается достичь стойкой ремиссии и контроля над заболеванием при помощи базисных противоревматических препаратов [5,6]. Угроза инвалидизации таких больных определяет необходимость разработки и внедрения новых подходов к патогенетической терапии, основанных на современ-465 ных медицинских технологиях и расшифровке фундамен-тальных механизмов развития болезни [7].Значительный прогресс в терапии ревматических болезней связан с внедрением в клиническую практику принципиального нового класса препаратов, объеди-ненных общим термином «генно-инженерные биологи-ческие препараты» (ГИБП) [8][9][10]. ГИБП -это проти-вовоспалительные лекарственные средства, которые производят с применением биотехнологий. Мишенями для ГИБП служат цитокины, их рецепторы, CD и кости-мулирующие молекулы и др. [11]. Основным преимуще-ством биологической терапии является максимальная избирательность воздействия на иммунную систему, позволяющая устранить необходимое звено в патогене-тической цепи, не воздействуя на клетки других органов и систем [12].Центральное звено в патогенезе ЮИА без систем-ных проявлений -фактор некроза опухоли (ФНО) ␣.
