Objective: To compare the remineralization potential of theobromine and sodium fluoride gels on artificial caries like lesion. Materials and Methods: Forty longitudinal halves of human mandibular premolars were equally divided into 4 groups of 10 samples each: control group (C), samples were stored in distilled water during the experiment period. The remaining 30 specimens were subjected to demineralization protocol to create caries like lesions. samples were immersed for three days in a demineralization solution (pH 5.0) containing 0.2% carbopol and 0.1% lactic acid saturated with calcium phosphate tribasic. The samples were subdivided into 3 equal groups according to the treatment applied during the pH cycle. Demineralization group “D”: no treatment applied. Group “F” treated with 2000 mg/liter sodium fluoride gel. Group “T” treated with 200 mg/liter theobromine gel. The specimens of the two studies groups were subjected to Demineralization- Remineralization PH Cycle Protocol for 5 days (Alternating four steps: 1: Treatment material, fluoride or theobromine ˜= 3 minutes. 2: Demineralizing solution 3 hours. 3: treatment material ˜= 3 minutes. 4: Remineralizing solution till the next cycle). The samples were investigated by scanning electron microscope (SEM) and energy dispersive x-ray analysis (EDXA). Results: The enamel of the demineralization group was porous with artificial caries like changes exposing the subsurface enamel rods with severe rod core defects. Theobromine gel and fluoride gel groups showed marked improvement in the surface characteristics in the enamel in both groups. Theobromine gel group showed more observable enamel surface improvement than the fluoride gel group. EDXA revealed that the calcium-phosphorus ratio displayed a descending order: (C > T > F > D). The differences between the two tested groups were not statistically significant. Conclusion: Theobromine gel had more effective remineralizing potential than fluoride gel as a result of its effect in improving the enamel surface characteristics of human teeth.KeywordsDental caries; Dental enamel, Ultrastructure; Hardness; Microscopy; Electron; Scanning; Sodium Fluoride, Therapeutic use; Theobromine/therapeutic use.

Introduction: By using fluoride, dental caries can be remineralized during the early stages of development. Although fluoride might be the first option for preventing cavities, too much fluoride exposure, particularly in children, can have negative effects, so we need alternatives. Due to their benefits, substances like theobromine and silver diamine fluoride are among the most hopeful ones for remineralizing demineralized enamel.Purpose: To compare between solution forms of theobromine and silver diamine fluoride in the remineralization potential of the demineralized human enamel.Materials & Methods: Twenty sounds mandibular first premolar were divided into 4 equal groups (5 samples each): group I Control group (C), group II demineralization group (D) subjected to demineralizing solution then 5 days PH cycle. The remaining two groups were similar to demineralization group with addition of treatment material, theobromine solution (T group) group III and silver diamine fluoride solution (S group) group IV, The samples were investigated by scanning electron microscope (SEM) and energy dispersive x-ray analysis (EDXA).Results: Demineralization group's enamel was porous, revealing underlying enamel rods with serious rod core defects. The enamel surface improved with the theobromine solution group, and silver diamine Fluoride solution also improved the enamel surface, but to a lesser extent than theobromine did. EDXA revealed that the calcium phosphorus ratio displayed a descending order: (C > T > S > D). the difference in the calcium to phosphorus ratio (CPR) of the studied groups.

Background: Primary tooth enamel are susceptible to damage because it has a low thickness so it requires preventing treatments to maintain. Theobromine, an alkaloid of the methylxanthine group, has been shown to improve crystallinity and resistance of enamel apatite solubility to acids. The enamel resistance to acid requires a series of test of physical properties, by measuring at microhardness. This study analyzes the effect of theobromine gel consentration on the hardness resistance of primary incisors enamel surfaces. Method: Twenty-five specimens were divided into 5 groups of topical application of theobromine gel with concentrations of 1%, 2%, 3%, 4%, and 5%, and then immersed in acidic solution. Results were analyzed using one-way ANOVA Result: One-way ANOVA test showed significant differences in the increase and decrease in enamel microhardness after the application of theobromine gel and acid immersion (p <0.05). The highest values of increase and decrease in enamel microhardness (764,74±187,08 VHN and 457,18±54,37 VHN, respectively) was shown in 5% group of theobromine gel. Conclusion:The higher theobromine gel concentration, the higher the microhardness value but unable to maintain the enamel resistance against acid. The 5% theobromine gel concentration was the most effective one to increase the microhardness of primary teeth enamel.
