Aim: In this experimental study, it was aimed to examine the effects of rocuronium and sugammadex complex on cardiac muscle cells in rats histopathologically and biochemically. Matherial and Method: 32 adult Sprague-Dawley male rats were divided into four groups with 8 in each group. Group 1 consisted of animals that did not undergo surgical treatment. Group 2 received a volume equivalent to 16 mg/kg sugammadex with 0.9% intravenous saline. Group 3 received 16 mg/kg of intravenous sugammadex. Group 4, 1 mg/kg intravenous rocuronium and 16 mg/kg intravenous sugammadex were administered to rats. After the procedure completed GSH and MDA level evaluated biochemically; and heart tissue evaluated histopathologically. Results: In group 4, connective tissue edema between muscle fibers was observed to be significantly increased, vessel dilatation and hemorrhagic areas were observed. Groups 3 and 4 were found to cause an increase in GSH level when compared to Groups 1 and 2, and a decrease in MDA level in these two groups compared to the others. Conclusion: Although sugammadex and sugammadex-rocuronıum complex cause biochemical and histopathological effect on the heart tissue, there were no irreversible histopathologic changes and no significant biochemical difference found in this study.

Previous studies have shown that medications from the cyclodextrin family bind to verapamil. The aim of our study was to determine whether sugammadex could bind to verapamil and prevent the cardiovascular toxicity of that drug. Twenty-eight sedated Wistar rats were infused with verapamil at 37.5 mg/kg/h. Five minutes after the start of infusion, the animals were treated with a bolus of either 16 mg/kg, 100 mg/kg or 1000 mg/kg sugammadex. The control group was treated with an infusion without sugammadex. The heart rate and respiratory rate were monitored, and an electrocardiogram was recorded. The primary end-point was the time to asystole. The verapamil infusion continued until the animals arrested. The asystole time for the S16 group was significantly longer compared to those for the control and S1000 groups (p < 0.05). The asystole time for the S1000 group was significantly shorter than those for all of the other groups (p < 0.05). Reflecting these data, there was a near doubling of the mean lethal dose of verapamil from 13.57 mg/kg (S.D. AE8.1) in the saline-treated rats to 22.42 mg/kg (S.D. AE9.9) in the sugammadex 16 group (p < 0.05). However, for the sugammadex 1000 group, the mean lethal dose was found to be 6.28 AE 1.11 mg/kg. This dose is significantly lower than those for all of the other groups (p < 0.05). We found that treatment with 16 mg/kg sugammadex delayed verapamil cardiotoxicity in rats. However, 1000 mg/kg sugammadex accelerated verapamil cardiotoxicity in rats. Further studies must be conducted to investigate the interaction between verapamil and sugammadex.

Mast cells play a vital role in hypersensitivity reactions. Rocuronium is known to cause mast cell mobilization, hypersensitivity, and pancreatitis. The aim of this study was to investigate the effects of sugammadex on pancreatic changes due to rocuronium. A total of 42 Sprague-Dawley male rats were divided into six equal groups to receive either rocuronium 1 mg/kg intravenously (i.v., R group), rocuronium 1 mg/kg + sugammadex 16 mg/kg i.v. (RS16 group), rocuronium 1 mg/kg + sugammadex 96 mg/kg i.v. (RS96 group), sugammadex 16 mg/kg (S16), sugammadex 96 mg/kg i.v. (S96 group), or 0.9% sodium chloride (control group). Sugammadex was administered 5s later following rocuronium. In R group, mast count was higher, and the distribution rate of granules and nuclear changes were different compared with other groups. Distribution rate of granules in groups S16 and S96 were similar to the control group and lower compared with other groups. The amount of mast cells and granule density in groups RS16 and RS96 was lower compared with R group. The amount of mast cells in groups RS16 and RS96 was significantly lower compared with other treatment groups. These results suggest that sugammadex may have an inhibitory effect on mobilization and morphological changes in pancreatic mast cells induced by administration of rocuronium and sugammadex in rats.
