IntroductionAdipose tissue exerts widespread effects on the metabolism and immune system, but its activity differs between the genders. In the general population low-grade adipose tissue inflammation contributes to development of diseases of affluence. Little is known about the systemic impact of peripheral fat tissue in osteoarthritis (OA) and rheumatoid arthritis (RA), characterized by chronic, low- and high-grade systemic inflammation, respectively. To clarify this we evaluated the secretory activity of subcutaneous abdominal adipose tissue (SAAT) obtained from male patients affected with RA (n = 21) and OA (n = 13), and assessed its association with body mass and composition, demographic, clinical and laboratory data.Material and methodsBasal and interleukin (IL)-1β-triggered secretion of selected adipocytokines from SAAT explants was measured by specific enzyme-linked immunosorbent assays (ELISA). Patients’ body composition was evaluated by bioelectric impendence technique.ResultsRheumatoid SAAT secreted more adiponectin and macrophage migration inhibitory factor (MIF) than respective osteoarthritis tissue. In both RA and OA patient groups, stimulation of SAAT explants with IL-1β (1 ng/ml/100 mg tissue) significantly up-regulated release of pro-(IL-6, IL-8, tumor necrosis factor – TNF) and anti-inflammatory (IL-10) cytokines but had no effect on the secretion of adiponectin, leptin, MIF and hepatocyte growth factor (HGF). Compared with RA, patients with OA were more obese. In RA patients SAAT-released adiponectin and TNF inversely correlated with body mass index (BMI) and visceral fat rating (FVSC). In addition, SAAT-secreted adiponectin and leptin positively correlated with DAS28 and disease duration, respectively. In the OA group tissue-released TNF positively correlated with patients’ age.ConclusionsWe conclude that in RA male patients adipocytokines originating from SAAT are of clinical importance because: (i) adiponectin and TNF may contribute to maintenance of normal body composition and mass, (ii) in addition adiponectin may play a pathogenic role. Moreover, in both RA and OA male patients secretory activity of SAAT may vary with time.

Adiponectin is the richest adipokine in human plasma, and it is mainly secreted from white adipose tissue. Adiponectin circulates in blood as high-molecular, middle-molecular, and low-molecular weight isoforms. Numerous studies have demonstrated its insulin-sensitizing, anti-atherogenic, and anti-inflammatory effects. Additionally, decreased serum levels of adiponectin is associated with chronic inflammation of metabolic disorders including Type 2 diabetes, obesity, and atherosclerosis. However, recent studies showed that adiponectin could have pro-inflammatory roles in patients with autoimmune diseases. In particular, its high serum level was positively associated with inflammation severity and pathological progression in rheumatoid arthritis, chronic kidney disease, and inflammatory bowel disease. Thus, adiponectin seems to have both pro-inflammatory and anti-inflammatory effects. This indirectly indicates that adiponectin has different physiological roles according to an isoform and effector tissue. Knowledge on the specific functions of isoforms would help develop potential anti-inflammatory therapeutics to target specific adiponectin isoforms against metabolic disorders and autoimmune diseases. This review summarizes the current roles of adiponectin in metabolic disorders and autoimmune diseases.

Concentración de adiponectina posterior a reducción de masa corporal en un grupo de mujeres obesas premenopáusicas en Lima, Perú

Investigar el cambio en la concentración de adiponectina circulante en plasma en 23 mujeres obesas premenopáusicas de la ciudad de Lima luego de la reducción de la masa corporal como resultado de un programa de actividad física aeróbica y una dieta baja en calorías. Materiales y métodos: Se llevó a cabo un estudio analítico y cuasi-experimental con el grupo de mujeres obesas. Adicionalmente, 24 mujeres se consideraron como control o de referencia de la concentración de adiponectina y otros marcadores bioquímicos. La cuantificación de adiponectina se realizó mediante la prueba de ELISA. La concentración de glucosa en ayunas en sangre, colesterol, lipoproteína de alta densidad y triglicéridos fueron cuantificados mediante análisis clínicos de rutina. Resultados: Antes del programa el grupo control mostró altos valores de adiponectina (mediana: 8,54 µg/mL; rango: 6,14 µg/mL a 13,49 µg/mL) en comparación con el grupo obesidad (mediana: 7,03 µg/mL, rango: 3;77 µg/mL a 17,23 µg/ mL); sin embargo, la diferencia no fue estadísticamente significativa (P = 0.0563). Luego de la finalización del programa se observó que el grupo obesidad presentó una reducción estadísticamente significativa del índice de masa corporal (P = 5.98e-08) y de la circunferencia abdominal (P = 1.55e-08) así como un incremento estadísticamente significativo de los niveles de adiponectina (mediana, 8.79 µg/mL; rango, 5.50 µg/mL a 19.37 µg/mL) (P = 0.0127). Conclusiones: Basándonos en los resultados, concluimos que en mujeres obesas premenopáusicas la concentración de adiponectina se incrementa cuando la masa corporal se reduce como resultado de actividad física aeróbica y una dieta baja en calorías.

Abstract: Adiponectin not only triggers pro-inflammatory and pro-destructive activities of rheumatoid FLS but also pre-disposes these cells to a stronger response to IL-1β. Thus, it is likely that adiponectin is more important in the initiation phase than in the chronic phase of RA.
