Abdominal as well as intra-visceral adhesions are considered to be one of the major postsurgical complications which is the results of inflammation, coagulation and fibrin depositions due to surgical injuries or irritations at the time of surgical intervention. In the present study, eighteen numbers of apparently healthy adult rabbits of either sex equally divided in to three groups were used to see the efficacy of Pectin-Honey Hydrogel and Olive oil-vitamin E preparation in prevention of induced intraperitoneal adhesions. Group A, Group B and Group C were treated with intraperitoneal application of normal saline, Pectin-Honey Hydrogel and Olive oil-vitamin E preparation respectively following induced peritonitis. Clinical parameters (0th 2nd, 4th, 6th, 8th and 10th days), biochemistry of peritoneal fluid (0th, 10th, 20th and 30th days) was carried out. The surgical wounds were reopened on 10th, 20th and 30th day for macroscopic assessment of adhesions. Clinical parameters found to be significantly elevated (P < 0.05) on 2nd post-surgical; however, the changes were within the physiological limit. Peritoneal fluid biochemistry revealed significant higher total protein concentration in Group A followed by Group C & B. A significantly lower catalase activity of peritoneal fluid was found in Group B post-surgically. Macroscopic peritoneal and visceral adhesion score was recorded highest in Group A and lowest in Group B. The present study revealed that Pectin-Honey Hydrogel and Olive oil-vitamin E preparation effectively prevent peritoneal adhesion in rabbits and promises its clinical use.

Hayat standartlarını olumsuz etkileyen abdominal adezyonlar, postoperatif dönemde görülen önemli bir sağlık sorunudur. Peritoneal kavite ve serozal yüzeylerde oluşan, abdominal travmalara sebep olan kimyasal ve termal faktörler ya da enfeksiyon ve yabancı cisim reaksiyonları adezyon oluşumuna sebep olabilir. Abdominal adezyonların sınıflandırması genellikle adezyon yoğunluğuna ve prognoz ciddiyetine göre yapılsa da henüz dünya çapında kabul görmüş standart bir sınıflandırma sistemi mevcut değildir. Abdominal adezyonlar ağrı, infertilite, cerrahi sonrası hastanede yatış süresinin uzaması ve ekonomik yük gibi olumsuz sonuçlarla klinik yansımalar gösterir. Sonuç olarak, postoperatif süreçte karşılaşılan adezyonlar ciddi bir sorundur ve adezyon oluşumunu engellemek için ileri çalışmaların laboratuvar ortamından klinik araştırma modellerine uyarlanması gerekmektedir. Bu derleme çalışması intraabdominal adezyon oluşumu, histopatolojisi, derecelendirilmesi, önlenmesi ve klinik önemi ile ilgili literatürü gözden geçirmek için hazırlanmıştır.

In the present study, the efficiency of intra-abdominal use of the macerated oil obtained from Taşköprü Garlic (Kastamonu/Türkiye), which is one of the products with geographical indication-from European Union, in preventing the postoperative adhesions. The chemical structure of the macerated-oil, prepared using fresh-garlic at a concentration of 1.2 g/ml in olive oil, was analyzed by spectrophotometric and chromatographic methods. In in vivo experiments, Wistar Albino rats (n=8) were applied cecal abrasion under general anesthesia. Abrasion zone was treated with physiological saline solution in control group (K) and different doses of macerated oil (0.5-1 ml) in study groups-(S1 S2). Macroscopic-adhesion-scoring-(MAS) between 0-4 was performed for all the subjects on postoperative days 3-28. After staining the tissue-samples, which were taken from subjects found to have adhesion, histopathological adhesion scoring (HAS) was performed between 0 3. On day 3, adhesion was detected in all the subjects. The adhesions reached MAS-2 level in the K and were in MAS 1 level in the subjects in S1 S2. On day 28, no adhesion was observed in S1-S2, whereas adhesions in K remained at the level MAS-2. Moreover, HAS were found to be HAS-1 in S1 S2-K on day 3 whereas it was found to be HAS-2 in K subjects, where adhesion formation continued, on day 28. As a result, the garlic-macerated-oil, obtained by soaking fresh garlic in olive oil for a time, is a low cost and effective agent for adhesion prevention, which will be easily produced and applied by clinicians.
