Purpose The purpose of this study was to investigate the effects of lysozyme, an antimicrobial enzyme found in tears that protects the eye against pathogens, on pseudotyped severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection through corneal epithelial cells. Methods The expression of the angiotensin-converting enzyme 2 (ACE2) and transmembrane serine protease (TMPRSS2) in human corneal epithelial cells (HCECs) was measured by RT-PCR and Western blotting. The altered expression of the pro-inflammatory molecules induced by spike protein and lysozyme was analyzed by RT-PCR. Cell toxicity was tested by CCK8 assay. The cell entry of SAR-CoV-2 in HCECs and primary rabbit corneal epithelial cells (RbCECs) was detected by luciferase assay. Results ACE2 and TMPRSS2 were highly expressed in HCECs. The spike proteins of SARS-CoV-2 stimulated a robust inflammatory response in HCECs, characterized by increased secretion of pro-inflammatory molecules, including IL-6, TNF-α, iNOS, and MCP-1, and pretreatment with lysozyme in HCECs markedly decreased the production of proinflammatory molecules induced by spike proteins. In addition, the inflammatory cytokine TNF-α enhanced the entry of SARS-CoV-2 into HCECs, which can be mitigated by pretreatment with lysozyme. Conclusions In this study, we analyzed the susceptibility of human corneal epithelial cells to SARS-CoV-2 infection and suggested the protective effects of lysozyme on SARS-CoV-2 infection.

Background: Meibomian gland disfunction is a common finding in glaucoma patients under topical treatment. Objective:To evaluate and analyze the Meibomian morphology in patients with different Glaucoma Topical Treatments. Methods: Cross-sectional retrospective study. 88 patients: 15 control and 73 glaucoma patients classified: a) Latanoprost-Timololcarbonic anhydrase inhibitor b) Latanoprost-Timolol, c) Timolol, d) Latanoprost, and e) Travoprost-Timolol (Polyquad). Slit-lamp examination and OSDI® questionnaire was evaluated. Glandular number, length, thickness, tortuosity percentage and tarsal/ glandular area (Meiboscore) was obtained from each Meibography. Results: Control presented the highest gland number (21.2 ± 5.3 upper and 17,3 ± 5,68 lower eyelid), longest glands (6,0 ± 0,94 mm upper and 3,2 ± 0,85 mm lower eyelid), thinnest glands (0,6 ± 0,14 mm upper and 0,8 ± 0,15 lower eyelid) and lower upper eyelid Meiboscore (63,3 ± 12,68%). Latanoprost-Timolol and carbonic anhydrase inhibitor presented the lowest tortuosity (27,1 ± 18,34% upper and 12,9 ± 13,99% lower eyelid). Latanoprost group presented the thickest glands (0,75 ± 0,10 mm upper and 0,94 ± 0,2 mm lower eyelid). The lowest Meiboscore in lower eyelid was Travoprost-Timolol (Polyquad) (59,7 ± 14,26%). Conclusions: Glaucoma Meibomian glands groups showed alterations compared to control. Polyquad showed less alterations in Meiboscore value than BAK groups, but not in all the parameters. Correlation with treatment but without cumulative preservative dose indicate another compounds effect. Upper eyelid glands seem to be affected earlier in these patients. Meibomian gland study should be considered as part of treatment decision and following in glaucoma patients.

Uncontrolled administration and prolonged use of antihypertensive drugs with preservatives often leads not only to subjectively poor tolerance of treatment, but also to a deterioration in the results of subsequent surgery, if required. The manifestations of dry eye syndrome and its aggravation as a result of such drops use can lead the patients to refuse therapy and worsening the prognosis. The manifestations of dry syndrome are often detected before any hypotensive therapy begins, since the functions of secretory cells change with age and there is always the influence of adverse environmental factors. It is necessary to examine the condition of the anterior surface of the eye to resolve the issue of the need to prescribe drugs without preservatives for all patients before the appointment of antihypertensive therapy. However, sometimes it is not possible due to lack of time or equipment. Therefore, it is important to study the condition of the anterior eye surface in patients with newly diagnosed glaucoma before and during treatment with nonpreservative forms of antihypertensive drugs in order to determine the necessity of the first prescription of non-preservative forms of antihypertensive drops to further interpolate its results into practical recommendations. This study revealed that drug treatment of patients with newly diagnosed glaucoma with the drug Timolol-POS® β-blocker on a non-preservative basis along with IOP compensation improves both subjective and objective indicators of the condition of the anterior surface of the eye with maximum effect after 3 months of treatment. This conclusion suggests the importance of the primary appointment for such patients with instillation drops on a preservative basis.
