Effect of Fluoride Varnish Combined with Er:YAG Laser on the Permeability of Eroded Dentin: An In Situ Study

This study evaluated the combined effect of fluoride varnish and Er:YAG laser on the permeability of eroded bovine root dentin. After initial erosive challenge followed by a remineralization period, the specimens were divided in two groups according to the treatment - fluoride varnish and non-fluoride varnish - and were subdivided according to the irradiation protocol: Er:YAG laser (100 mJ, 3 Hz, 12.8 J/cm2per pulse, non-contact and defocus mode) and non-irradiated. After a lead-in period, 7 volunteers wore a palatal device containing 4 specimens that were subjected to erosive challenges. At the first experimental phase, 4 volunteers used specimens treated with fluoride varnish and fluoride varnish+Er:YAG laser and 3 volunteers used specimens treated with non-fluoride varnish and non-fluoride varnish+Er:YAG laser. After a washout period, volunteers were crossed to treatments, characterizing a 2x2 crossover experiment. At the end of the experimental phase, the quantitative response variable was obtained by permeability analysis and the qualitative response by scanning electron microscopy (SEM). Two-way ANOVA and Tukey-Kramer's test revealed that specimens treated with fluoride varnish+Er:YAG laser showed the lowest permeability and a significant difference was found between this group and the others. When varnish (fluoride/non-fluoride) was applied in the absence of Er:YAG laser, higher permeability was found when compared to the laser-treated groups. SEM evaluations showed partially or completely obliterated dentinal tubules when specimens were treated with fluoride varnish+Er:YAG laser. It may be concluded that Er:YAG laser was able to control the permeability of eroded root dentin and the combination with fluoride varnish increased laser action.

ResumoIntrodução: A erosão dentária caracteriza-se como uma perda progressiva de estrutura dentária em que ocorre dissolução localizada de minerais da superfície do dente, ocasionada devido a exposição a ácidos de origem não bacteriana. Esse agravo bucal tem ganhado destaque pelo aumento de sua prevalência, que tem ocorrido, principalmente, devido a mudança dos hábitos alimentares e no estilo de vida da sociedade moderna. Objetivo: Apresentar uma síntese da ação das pastas de CPP-ACP no controle da erosão dentária. Material e Método: Foi realizada uma revisão da literatura nas bases de dados eletrônicas: Pubmed, Scopus e Bireme, utilizando os termos Erosão Dentária, Tooth Erosion, Esmalte Dentário, Dental Enamel, Caseínas, Caseins, Fluoreto de Sódio, Sodium Fluoride e os operadores boleanos "AND" e "OR" para realização das buscas, não havendo restrição quanto ao ano ou idioma de publicação. Resultados: Diferentes condutas terapêuticas têm sido avaliadas como potenciais alternativas para o controle da erosão dentária, dentre os quais está o uso da caseína fosfopeptídeo -fosfato de cálcio amorfo (CPP-ACP). As pastas de CPP-ACP parecem ser o tipo de veículo com maior potencial preventivo, além disso, a literatura sugere também, que as abordagens adotadas previamente às exposições ácidas tendem a ser mais bem-sucedidas. Apesar disso, ainda não há um consenso sobre a eficácia desse recurso terapêutico, devido, principalmente a carência de estudos realizados sob protocolos in situ. Conclusão: A ação preventiva das pastas de CPP-ACP, suplementadas ou não pelo fluoreto, permanece incipiente na literatura. Background: Dentin sensitivity is one of the most important problems in dentistry. Enamel loss due to root exposure is serious issue and common exposure is one of the reasons for dentin hypersensitivity. There are different methods for solving this problem. One of the most conservative and least expensive methods is use of casein phosphopeptide-amorphous calcium phosphate (CPP-ACP) paste. Objectives: The aim of this study was to evaluate shear bond strength of GIC to dentin, with or without laser, CPP-ACP paste and polyacrylic acid treatments. Materials and Methods: Fifty sound human third molars were bisected in a mesiodistal direction using a diamond disk. Using 400, 600 and 800 grit silicon carbide paper, dentin surfaces were exposed. The teeth were divided into five groups. In groups A, B, D and H, CPP-ACP (GC tooth mousse Itabashi-Ku, Tokyo, Japan) was applied for one hour the first day and repeated at the same time of day for a total of five days. In groups B, C, D and E, the specimens were subjected to laser for 10 seconds using Er, Cr: YSGG laser. In groups B, C, H and G, specimens were treated with 10% polyacrylic acid for 20 seconds. A plastic tube containing GI was positioned over the tooth. Samples were loaded in shear bond using a Universal Testing Machine (Zwick/Roell, Germany), at a 0.5 mm/minute crosshead speed. Results: Despite the failing of groups A and D, group analysis showed that there were no significant differences between the groups. The predominant type of fracture in all groups was adhesive. Conclusions: Application of CPP-ACP, without preconditioning with polyacrylic acid, can decrease shear bond strength. Laser irradiation has no effect on shear bond strength of GIC to dentin in this condition.
