Purpose: Description of cognitive behavioural therapy (CBT) of a patient diagnosed with paranoid schizophrenia, who experienced chronic negative auditory hallucinations (voices). Case description: Elements of CBT complemented by the use of virtual reality were described. Stages of the therapy of a 40-yearold patient hearing voices constantly for 7 years were presented. During the therapy, the patient was still treated pharmacologically (olanzapine 20 mg/day and risperidone 2 mg/day). Conceptualisation of the problem, therapy plan and the course of selected therapeutic interventions using an avatar were discussed. The empty chair technique supported by the use of the avatar was presented. The effects of 20 therapeutic sessions were evaluated, confirming a significant reduction in the frequency and intrusiveness of voices and sustainability of these effects in the 6-month follow-up. Comment: The described case is an example of a short-term effective therapeutic intervention during which, at the cognitive level, changes were observed in the experienced auditory hallucinations. Thanks to the use of the avatar, it was possible to create a representation of auditory hallucinations and to give them physical features, and, consequently, to work on changing beliefs about the source of these hallucinations. Furthermore, during the therapy, the avatar served as an exponent of critical content which the patient experienced in relations with his father. He was referred by a psychiatrist as there were no more pharmacological treatment options available. The patient often reported significant severity of the experienced hallucinations, which initially resulted in frequent changes of medications, usually increasing the dose and adding other antipsychotics. It created a sense of helplessness on the part of both the patient and the doctor.The therapy was given in the Mental Health Outpatient Clinic of the Institute of Psychiatry and Neurology in Warszawa. Sessions were held regularly every week. Each session lasted 50 minutes.The patient continued his pharmacological treatment (olanzapine 20 mg, risperidone 2 mg) which remained unchanged during the therapy. This paper explores the experiences of women who "hear voices" (auditory verbal hallucinations). We begin by examining historical understandings of women hearing voices, showing these have been driven by androcentric theories of how women's bodies functioned leading to women being viewed as requiring their voices be interpreted by men. We show the twentieth century was associated with recognition that the mental violation of women's minds (represented by some voice-hearing) was often a consequence of the physical violation of women's bodies. We next report the results of a qualitative study into voice-hearing women's experiences (n = 8). This found similarities between women's relationships with their voices and their relationships with others and the wider social context. Finally, we present results from a quantitative study comparing voice-hearing in women (n = 65) and men (n = 132) in a psychiatric setting. Women were more likely than men to have certain forms of voice-hearing (voices conversing) and to have antecedent events of trauma, physical illness, and relationship problems. Voices identified as female may have more positive affect than male voices. We conclude that women voice-hearers have and continue to face specific challenges necessitating research and activism, and hope this paper will act as a stimulus to such work.

The experience of hearing voices is common to an estimated 7% of the general population, with the presence of negative content being the best predictor of whether individuals will require clinical support. Whilst largely neglected in the literature to date, there are calls to consider the significance of voice content for reducing voice‐related distress. However, no quantitative and comprehensive measure of voice content with suitability for research and clinical use exists. This pilot study aimed to demonstrate preliminary psychometric properties of a newly developed measure, the Perth Voice Content Questionnaire (PVCQ), designed to measure the intensity and themes of positively and negatively valenced voice content, primarily of the dominant voice. The PVCQ and measures of voice severity and related beliefs were completed by 47 voice‐hearing participants. The measure was found to be internally consistent, loaded onto two distinct factors of positive and negative content, and these factors were associated with voice‐related distress and negative beliefs about voices and positive beliefs about voices, respectively, indicating good validity. The PVCQ offers the first self‐report measure of voice content, with preliminary psychometric properties indicating its suitability for clinical and research use.
