What Is Known and Objective: Chronic myeloid leukaemia (CML) microenvironment is responsible for resistance of leukaemic cells to tyrosine kinase inhibitor, altered adhesion, increased proliferation and leukaemic cells growth and survival through the secretion of many soluble molecules. We aimed at monitoring soluble L-selectin (sCD62L) and secreted protein acidic and rich in cysteine (SPARC) levels in chronic phase chronic myeloid leukaemia (CP-CML) patients and assessing the impact of imatinib on these parameters.Methods: This prospective controlled clinical trial enrolled 35 subjects classified into two groups: control group included 10 healthy volunteers and CP-CML patients group included 25 newly diagnosed CP-CML patients received imatinib 400 mg once daily. sCD62L plasma levels, SPARC serum levels, breakpoint cluster region-Abelson1 (BCR-ABL1) %, complete blood count with differential, liver and kidney functions parameters were assessed at baseline and after 3 and 6 months of treatment.Results and Discussion: At baseline, sCD62L and SPARC were significantly elevated in CP-CML patients (p < 0.05) compared to control group. After 3 months of treatment, sCD62L was non-significantly decreased (p > 0.05), while surprisingly SPARC was significantly increased (p < 0.05) compared to baseline. Moreover, after 6 months of treatment, sCD62L was significantly decreased (p < 0.05) and SPARC was non-significantly decreased (p > 0.05) compared to baseline. In addition, sCD62L was significantly correlated with WBCs and neutrophils counts, while SPARC was significantly correlated with lymphocytes count at baseline and after 3 and 6 months of imatinib treatment. What Is New and Conclusion:The elevated levels of sCD62L and SPARC at diagnosis in CP-CML patients could reflect their roles in CML pathogenesis and the dynamic changes in their levels during imatinib therapy might suppose additional mechanisms of action of imatinib beside inhibition of BCR-ABL. Furthermore, imatinib showed a significant impact on sCD62L and SPARC levels during treatment period.

Influence of Increased sCD56 and Scd16 Serum Concentrations on the Secretory and Cytotoxic Activities of Peripheral Bloodmononuclear Cells in Healthy People Living in Arctic Russia

Objectives. The CD56 differentiation cluster has been found on cells of neurogenic origin, muscle, natural killer cells, and on various populations of T lymphocytes, neutrophils, monocytes, epithelial and dendritic cells; this cluster sends signals to activate the secretion of chemokines, integrins, enzymes, cytokines, and other biologically active substances that are characteristic for this cell type. The CD16 third receptor for Fc immunoglobulins (FcγRIII) has been found on the membranes of granulocytes, mast cells, monocytes, macrophages, natural killers, and lymphocytes. The Fc fragment of antibodies provides the cytophilicity of immunoglobulins and frequently binds to IgM, IgA, and IgE. The aim of this study was to evaluate the effect of elevated concentrations of sCD56 and sCD16 on the secretory and cytotoxic activity of peripheral blood mononuclear cells in healthy people. Methods. An immunological examination was performed on 178 healthy people aged 21 to 55 years, including 112 women and 66 men, living in the Murmansk region and on the Spitsbergen (Svalbard) archipelago. The peripheral venous blood was analyzed; specifically, the hemogram, phagocytic activity of neutrophilic leukocytes, and contents of lymphocyte phenotypes (CD3+, CD4+, CD8+, CD10+, CD16+, CD56+, CD25+, CD71+, HLADRII, CD95+) were determined using an Epics XL instrument (Beckman Coulter, USA). The concentrations of free sCD16, sCD56, cytokines IL-1β, IL-6, TNF-α, IL-10, cGMP and cAMP, and circulating immune complexes (CICs) to IgA, IgM, and IgG were evaluated using an Evolis automatic analyzer (Bio-Rad, Germany). Results. Increases in the serum concentrations of sCD56 and sCD16 in healthy adults of working age are associated with an increase in the content of circulating mononuclear cells with the corresponding membrane clusters. An increase in sCD16 concentration is associated with a decrease in circulating leukocytes in the blood, and an increase in sCD56 concentration is associated with an increase in cytokines. Concentrations of CICs increase in parallel with increases in sCD16 and sCD56 concentrations. An increase in signal-sending activity with an increase in circulating CD16 + and CD56 + leukocytes and the shedding of these signaling molecules occur in parallel with decreases in the cAMP and cGMP concentrations. Conclusion. The increase in serum concentrations of sCD56 and sCD16 reflects the specificity and activity of the functions they perform. Membrane CD56 sends a signal that activates cell secretory activity, which results in increases in serum cytokine and immunoglobulin concentrations. In addition, membrane CD56 initiates cell interaction with immunoglobulins. The interaction of mCD16 and sCD16 with immunoglobulins causes a decrease in circulating lymphocytes in the blood and an increase in serum CIC concentration. The activation of signal transduction through CD56 and CD16 and their shedding occur in parallel with a twofold decrease in the extracellular cAMP and cGMP concentrations.

The paper presents the results of studying the immunological parameters of 369 people who were practically healthy at the time of the survey, 298 women and 71 men, of which 216 people are living in the European North of the Russian Federation (173 women and 43 men) and 153 are residents of the Arctic (125 women and 28 men). The study was carried out in the morning (08:00–10:00 am). The study included the determination of the aggregation of erythrocytes, platelets, neutrophilic granulocytes, lymphocytes, hemogram study, hematological analysis, enzyme immunoassay, and flow cytometry. Statistical processing of the obtained data was carried out using the Statistica 7.0 software package (StatSoft, USA). It was found that the activity of aggregation of cells of peripheral venous blood in Arctic residents is 1.5–1.7 times higher than that in people living in more favourable climatic conditions. The frequency of registration of aggregation of erythrocytes and platelets is actually 2 times higher than the aggregation of leukocytes. Aggregation of erythrocytes is associated with an increase in the concentrations of transferrin and receptors for this transport protein. The frequency of detection of platelet aggregation is accompanied by an increase in transferrin concentrations; in cases of aggregation of nonnuclear blood cells, the content of NO2 in the blood serum is increased. Aggregation of neutrophilic granulocytes and lymphocytes is associated with an increase in the content of free adhesion molecules. Aggregation of erythrocytes and platelets is in evidence when it is necessary to trigger reactions of changes in the hemodynamics of microcirculation to increase the efficiency of oxygen and trophic supply of tissues. The adhesion of leukocytes to the endothelium determines the secretion of biologically active substances that contribute to a change in microcirculation and an increase in the migration of leukocytes into tissues for the implementation of phagocytic and cytolytic functions.
