Gingival recession defect (GRD) may be defined as an apical migration of the gingival margin respective to the cementoenamel junction resulting in partial exposure of the root surface to the oral cavity, which may have important esthetic, functional, and periodontal health implications. A novel system for the classification and management of non-proximal GRDs is proposed in this article. This evidence-based system consists of two essential components: (1) Establishment of the GRD type based on the midbuccal/midlingual attachment level respective to the interproximal bone level, and (2) Assessment of the gingival phenotype according to the width of attached gingiva and gingival thickness. Each category of this new classification system is linked with treatment recommendations substantiated by relevant literature pertaining to the outcomes of validated root coverage procedures in specific scenarios, which can be used as a guide for clinical decision-making in daily practice.

The field of dentistry has evolved where people expect the best oral health care from specialists. Periodontics is that specialty of dentistry which deals with prevention, diagnosis, and treatment of diseases of the supporting tissues of the teeth. Almost half of the world's population is suffering from periodontal diseases. Periodontitis is the sixth most common chronic diseases in the world and along with dental caries, the most common cause of tooth loss. Through the effort of various professional organisations and research, various reliable products and treatment modalities have been developed. A gold standard is a benchmark which has been thoroughly tested and has reputation as a reliable modality. Some of established gold standards in periodontics include: periodontal probing, measurement of clinical attachment loss, bone loss, cone beam computed tomography, quantitative polymerase chain reaction tests, biopsies as investigative techniques; periodontal debridement, subepithelial connective tissue graft for recession coverage, lasers, autogenous bone in alveolar ridge augmentation prior to oral implantation, dental implant as reconstruction of missing dentition, and chlorhexidine mouth wash as treatment options. The objective of the review is to provide critical evaluation of the data available from existing studies in Periodontics which can help identify potential research areas to explore.

Gingival plasma cell granuloma (GPCG) is a rare benign, reactive, non-neoplastic lesion characterized by the predominance of polyclonal plasma cells. Clinical diagnosis is often confused with peripheral giant cell lesion, plasmacytoma, lymphoma, and fibroma. Surgery resection predominates the treatment methods. Routine resection extends to adjacent normal tissue, causing postoperative aesthetic and soft tissue defects. A 58-year-old female presented an uncommon maxillary anterior gingival mass. The clinical examination revealed that it was red in color, exophytic, painless, lobulated, pedunculated, and prone to bleeding. It has no systemic symptoms. Four weeks after scaling and root planning, the lesion showed a good response to periodontal initial therapy. Then it was treated by immediate laterally positioned flap (LPF) after biopsy. The sections stained with hematoxylin and eosin showed that the subepithelial tissue contained numerous granulation tissues with lymphocytes and plasma cells infiltration, suggesting a plasma cell lesion.Immunohistochemistry staining of the tissue section showed notable cytoplasmic positivity for both κ and λ light chains. The pathological diagnosis was GPCG. No recurrence was seen at the 6-month follow-up.The technique of immediate LPF after biopsy is speculated as an alternative to simple resection of GPCG. It could yield satisfactory esthetic outcomes and prevent postoperative root sensitivity and pain.
