The Combined Utility of HBME-1 and Galectin-3 Immunohistochemistry and BRAF V600E Mutations in the Diagnosis of Papillary Thyroid Carcinoma

Abstract: Newer diagnostic modalities have revolutionized the pathologist’s approach to diagnosing thyroid malignancies. Molecular characterization of these malignancies has helped circumvent common morphologic diagnostic difficulties by integrating their genotypic, phenotypic, and immunohistochemical features. BRAF V600E mutation has been characterized as highly specific for thyroid carcinoma, especially papillary thyroid carcinoma (PTC); human bone marrow endothelial marker-1 (HBME-1) and galect… Show more

“…However, none of these antibodies presented enough sensitivity and specificity to be used in routine practice. 78 , 79 , 80 , 81 , 82 , 83 In recent years, a specific monoclonal antibody (Ve1) against the mutated V600E BRAF protein began to be used for differential diagnosis and patient management. Few reports on the cytological application-assisted application of BRAF Ve1 antibody suggest that immunohistochemistry can be performed more reliably in cell block preparations, and false-positive results in direct smears may limit the benefit; thus, caution should be exercised in interpretation.…”

“…Interesting results came from the correlation between HBME-1 immunohistochemical expression and genetic mutations, with a trend noted in follicular carcinomas with peroxisome proliferator-activated receptor (PPAR) rearrangements that fail to show HBME-1 immunoreactivity in 59% of cases, in contrast to RAS mutated cases that are often positive (62% of cases) 28. This strict relation with the underlying genetic background of thyroid neoplasms culminates in PTC, in which a direct positive correlation between BRAFV600E mutation and HBME-1 expression has been noted 29…”
