Accelerating orthodontic tooth movement has become a topical issue and the corticotomy seems to be the only effective and safe technique reported in the literature. Simultaneously, aesthetic orthodontic treatment with removable clear aligners has become commonly requested. The aim of this paper is to illustrate the management of facilitated aesthetic orthodontic treatment, a combined approach including piezocision corticotomy and clear aligners for orthodontic treatment. Orthodontic planning for traditional clear aligners should be modified to take advantage of the corticotomy technique in order to facilitate the most difficult orthodontic movements needed to achieve treatment completion, where each aligner will be used for four days rather than 15 days for a total time of four months. A corticotomy with a modified minimally invasive flapless piezocision technique should be performed in both jaws at the same time, before the time window of the orthodontic treatment, where the most difficult orthodontic movements are planned. Treatment planning where difficult orthodontic movements, such as anterior open-bite closure and extraction space closure, are easily managed with clear aligners and are presented as examples of facilitated aesthetic orthodontic treatment application. The combination between aesthetic treatment with clear aligners and modified piezocision corticotomy, if carefully planned, seems to represent a synergy that achieves the current goals of orthodontic treatment. The primary objectives of this combination should be facilitating difficult orthodontic movements and reducing treatment duration.

Aim: This paper aims to assess the evidence in the literature reporting orthodontically induced inflammatory root resorption (OIIRR) in treatment with orthodontic clear aligners using 3D measurements. Materials and methods: Following preferred reporting Items for systematic reviews (PRISMA) statement, eight electronic databases were searched for relevant published and unpublished records. Data collected according to restricted inclusion and exclusion criteria. Results: A total of 236 articles were identified as relevant to our topic. Duplicates were excluded resulting in 226 papers, out of which 31 papers were relevant after screening titles and abstracts. Only 2 high-level evidence papers out of the 31 met the inclusion criteria for the qualitative synthesis. Conclusion: Based on the available studies with high level of evidence in the literature, we conclude that orthodontic clear aligners are non-inferior to light-force fixed orthodontic appliances, and superior to heavy-force fixed orthodontic appliances in terms of the risk for developing apical root resorption. Clinical significance: Orthodontists can be more assured about the low-risk of OIIRR associated with clear aligners compared to other orthodontic treatment modalities, and it remains up to the practitioner's assessment to select the appropriate treatment on a case by case basis.

Technological progress and the introduction of modern therapeutic methods are constantly changing contemporary orthodontics. More and more orthodontic patients are working adults, who expect satisfactory therapeutic effects as soon as possible, increasing the importance of methods accelerating tooth movement. The aim of this study was to review the current literature regarding methods of accelerating tooth movement and reducing the duration of the active phase of therapy. The literature was collected from the PubMed and EBSCO databases using "accelerated orthodontic tooth movement" as the search key words. The methods described were categorized as conservative and surgical. The pharmacological agents used in conservative treatment, such as growth hormone, parathyroid hormone, thyroxine, and vitamin D, are especially worth mentioning. They stimulate osteoclasts to increase resorption through a variety of mechanisms. Effective methods also include physical stimuli, e.g., vibrations or photobiomodulation. Most studies describing the effects of pharmacological agents were based on animal subjects and they may therefore lack clinical relevancy. Corticotomy and its modifications based on the regional acceleratory phenomenon (RAP) might prove to be a useful augmentation of orthodontic treatment, especially in adults, including patients with periodontal disease.
