The Clinical Course of Coronavirus Disease 2019 in a US Hospital System: A Multistate Analysis

Abstract: There is limited data on longitudinal outcomes for COVID-19 hospitalizations that account for transitions between clinical states over time. Using electronic health record data from a St. Louis-region hospital network, we performed multi-state analyses to examine longitudinal transitions and outcomes among hospitalized adults with laboratory-confirmed COVID-19 with respect to fifteen mutually-exclusive clinical states. Between March 15 and July 25, 2020, 1,577 patients were hospitalized with COVID-19 (49.9% ma… Show more

“…The coronavirus disease 2019 has a wide clinical spectrum, with disease severities ranging from completely asymptomatic to the need for intubation and death [1][2][3][4] . For example, those with existing cardiac problems are more likely to suffer from more severe disease life courses [5][6][7][8][9][10][11] , with potential modifier effects from different medication classes [12][13][14] .…”

“…Furthermore, death is the only end event considered. [ 1 , 2 , 7 11 ] also consider multistate survival models with different predictors, but only with hospitalised patients and, as a consequence, cannot give any assessment of the hospitalisation risk for the general population. This comment applies also to [ 22 ], published after the submission of the first version of the present paper.…”

“…The clinical course of Covid-19 is characterized by an initial period of mild to moderate symptoms, followed by progressive respiratory failure and requirement for intensive care unit (ICU)-level organ support or death in some patients. 1,2 The majority of patients requiring hospitalization are moderately ill, not initially requiring ICU-level organ support. 3-5 Limited therapies are available to prevent clinical progression to ICU-level organ support and death among moderately ill hospitalized patients.…”
