La LFR ha sido descrita con diversos nombres como: linfangioma renal, linfangiectasia peri-pélvica, enfermedad poliquística del seno renal, higroma renal y linfangiectasia renal peripélvica multiquística.Se cree que ocurre por una alteración de la comunicación entre los conductos linfáticos renales y los linfáticos retroperitoneales 1 . Comunicamos el caso de una mujer con LFR unilateral, en que esta alteración se descubrió incidentalmente en el estudio de un dolor abdominal asociado a hematuria microscópica. Caso clínicoMujer de 48 años, portadora de una válvula aórtica bicúspide con una leve insuficiencia, acné rosácea y colecistectomizada. Consultó en el servicio de urgencia por dolor abdominal localizado en el flanco izquierdo. El examen de orina mostró 7-10 hematíes por campo, leucocitos 4-6 por campo y ausencia de bacterias. Nitrógeno ureico 21,8 mg/dl, creatinina plasmática 0,59 mg/dl, clearance de creatinina de 24 h 109,7 ml/min/1.73m 2 y proteinuria de 24 h negativa. Electrolitos plasmáticos normales, glicemia 89 mg/dl, proteína C reactiva < 5 mg/L (VN: 0,10-8,2). Hematocrito 40,3%, hemoglobina 13,8 g/dl, leucocitos 7.400 mm 3 , plaquetas 283.000 mm 3 y VHS 1 mm/h. Por el dolor y la hematuria se efectuó un scanner de abdomen que mostró una masa quística multilocular en el hilio del riñón izquierdo, que protruye hacia el retroperitoneo y que no se realzaba con el uso del medio de contraste (Figura 1). Se diagnosticó un linfangioma quístico renal izquierdo. Como desapareció el dolor con terapia analgésica, se decidió no intervenir y continuar una observación anual. A 43-year-old man was referred to our hospital for an acute-onset fever and left flank pain. He had been previously diagnosed with lymphangioma, and abdominal computed tomography showed pararenal cysts with fat stranding around the left kidney, of which infection was subsequently confirmed on magnetic resonance imaging. Gram-negative spiral bacilli were isolated from two sets of blood cultures, and Helicobacter cinaedi was identified using 16S rRNA sequencing. The patient was successfully treated with ceftriaxone therapy without recurrence. A multilocus sequence typing analysis indicated the current H. cinaedi strain differed from previous strains isolated in Japan.
