The Cardiovascular Health in Ambulatory Care Research Team (CANHEART)

Abstract: T he CArdiovascular HEalth in Ambulatory careResearch Team (CANHEART) was formed in 2012 to measure and improve the cardiovascular health and quality of ambulatory cardiovascular care provided to the adult population of Ontario, Canada. “…The study population was derived from the Cardiovascular Health in Ambulatory Care Research Team (CANHEART) cohort (http://www.canheart.ca), which comprised the linkage of multiple individual‐level databases using encoded personal identifiers 11.
