Modern cataract surgery is safe in more than 95 per cent of patients. In the small number of cases where a serious complication occurs, the most common is an intra-operative posterior capsular rupture. This can lead to vitreous loss or a dropped nucleus and can increase the risk of post-operative cystoid macular oedema or retinal detachment. Postoperatively, posterior capsular opacification is the most common complication and can be readily treated with a YAG capsulotomy. The most devastating complication is endophthalmitis, the rate of which is now significantly decreased through the use of intracameral antibiotics. As a clinician, the most important step is to assess the patient pre-operatively to predict higher risk individuals and to counsel them appropriately. In these patients, various pre-or intra-operative management steps can be taken in addition to routine phacoemulsification to optimise their visual outcome.

RESUMENObjetivos: Valorar experimentalmente la integridad morfológica de los ligamentos suspensorios de la zónula de Zinn (LSZZ) tras la introducción de un anillo de distensión capsular (ADC) mediante un estudio descriptivo. Métodos: Se utilizaron siete ojos de cerdo a los cuales se realizó una extracción extracapsular de cristalino transparente. A cuatro se introdujo un ADC con pinzas y al quinto con inyector. Los otros dos se utilizaron como control. Se examinó la integridad anatómica y la nueva configuración de los LSZZ con un microscopio electrónico de barrido. Resultados: No se observaron alteraciones en la integridad de los LSZZ. Los LSZZ adoptan tras la introducción de un ADC una nueva trayectoria de repliegue consecuencia de la nueva disposición del saco capsular. Conclusiones: En un modelo experimental con ojos de cerdo no se encontraron lesiones en los LSZZ por lo que parece que, en cápsulas elásticas y ARTÍCULO ORIGINAL ABSTRACT Purpose: To carry out an experimental and descriptive study that exhibits the anatomical repercussions on the zonular apparatus after a capsular tension ring (CTR) is inserted. Methods: CTRs were inserted in five swine eyes (four with forceps, one with an injector). Two additional eyes were left untouched for control purposes. The integrity of the suspensory ligament was examined by scanning electron microscopy. Results: We did not observe alterations in the integrity of the zonular apparatus. The suspensory ligaments adopted a new fold configuration. Conclusions: CTR insertion is a safe therapeutic tool to maintain the integrity of the lens zonules in elastic, large diameter capsular bags (Arch Soc Esp Oftalmol 2008; 83: 589-594).

Purpose: To evaluate the resistance to compression of commercially available capsular tension rings (CTRs) when submitted "in vitro" to a constant pressure over a period of time.Setting: Research Unit. CHU Albacete. University of Castilla La Mancha. Spain.Method: Four samples of 3 different CTR manufacturers were evaluated for resistance to compression over time while being exposed to a constant tension. The 13 millimetres CTRs were placed in a round rigid container with an 11 millimetres diameter and submerged in BSS at body temperature (37 degrees Celsius). CTRs were divided into two groups. Two samples of each model were introduced in the containers and removed and evaluated at 1 month, while the other two samples of each model were introduced in the containers and removed and evaluated at three months. Size and shape of the CTRs were measured at baseline, at 1 month and 3 months. Every measurement was repeated 3 times by two different observers. Results:The mean decrease in CTRs diameter of the 1 month compression group was 1, 5 millimetres. The mean decrease in CTRs diameter of the 3 month compression group was 1, 63 millimetres. The differences in diameter between baseline and 1 month, as well as baseline and 3 months were statistically significant. There was no statistical significance when comparing the final size between the 1 month and 3 months groups. Conclusion:All the commercially available CTRs studied have shown a decrease in diameter when exposed to a constant compression at body temperature. This could explain some of the published reports of capsular bag shrinkage or even dislocation of the entire IOL-CTR complex despite the implantation of a CTR.
