The BNP assay does not identify mild left ventricular diastolic dysfunction in asymptomatic diabetic patients

Abstract: Aims: We examined the usefulness of BNP for screening for left ventricular (LV) diastolic dysfunction in a sample of type 2 diabetic patients, without structural heart disorder, who have never presented symptoms or signs of heart failure (HF). Methods and results: Seventy-six consecutive patients admitted to the Outpatient Diabetes Clinic were studied. Blood samples were analyzed using the Triage BNP fluorescence immunoassay (Biosite Diagnostics, La Jolla, CA, USA). Echocardiography examinations were performed… Show more

“…For instance, some groups have noted that BNP can reliably screen patients with diabetes for the presence or absence of left ventricular dysfunction even in the absence of clear indications for echocardiography such as symptoms (82,83). However, a number of other groups have suggested that BNP is not capable of identifying mild left ventricular diastolic dysfunction in asymptomatic patients with diabetes, particularly when these patients do not have evidence of macrovascular disease (84,85). Moreover, kidney disease per se, in the absence of either diabetes or HF, results in elevated BNP, adding further difficulties in the interpretation of these clinical studies.…”

“…Boyer et al [20] diagnosed diastolic dysfunction in 47% of asymptomatic patients with type 2 diabetes but with normal blood pressure values, while Valle et al [21] documented subclinical LV diastolic dysfunction in 51.3% of diabetics with normal LV systolic function.…”

“…Ассоциация уровня BNP с диастолической функ-цией ЛЖ, кажется, особенно сильно выражена у па-циентов с СД [14]. Диагностическая роль BNP в раннем выявлении бессимптомной диастолической дисфунк-ции у больных ХСН и СД 2 типа по-прежнему остает-ся спорной [6,18,[21][22][23][24][25][26].…”
