The biochemical utility of chromogranin A, chromogranin B and cocaine- and amphetamine-regulated transcript for neuroendocrine neoplasia

Abstract: Neuroendocrine neoplasia (NEN) is a heterogeneous group of tumours and often represents a therapeutic challenge to clinicians. The peptides chromogranin A (CgA), chromogranin B (CgB) and cocaine-and amphetamine-regulated transcript (CART) are widely distributed throughout the neuroendocrine system. CgA and CgB have been used as general NEN biomarkers for many years, while CART has only recently been identified. Of these biomarkers, CgA is the most commonly used. However, circulating CgA concentrations exhibit … Show more

“…Therefore, "cutoff levels" recommended for decision-making should be applied with caution, and each institution has to define its own "cutoff levels." Chromogranin B Plasma chromogranin B (CgB) may be of value in the follow-up of BP and rectal NEN [18] . However, the availability for CgB is limited.…”

“…Other PET tracers, such as 64 Cu-DOTATATE-PET-CT [30,31] , 18 In-DTPA-exendin-4 SPECT-CT [37] , may be used in follow-up if positive at baseline and if available at the center. Currently, there are only limited data available of PET-based imaging for routine follow-up, and its significance in this respect is currently not possible to define.…”

“…In a comparative study of 44 patients, CgA was expressed in nearly 100%, CgB was present in 88% and CgC was only present in 6%. A commercial assay is available for circulating CgB which, unlike that used for CgA, is not affected by PPIs or impaired kidney function [62]. …”

“…The determination of circulating chromogranin A (CgA), based on the secretory nature of GEP-NENs, has until now generally been considered the most useful biomarker. However, CgA is not highly specific to GEP-NENs, since it can be found in other malignancies and other non-tumor-related conditions such as chronic atrophic gastritis, proton pump inhibitor (PPI) therapy, renal insufficiency, systemic hypertension and severe liver failure [7,8]. …”
