The application value of lung ultrasound findings in preterm infants with bronchopulmonary dysplasia

Abstract: Background: The purpose of this study was to investigate the association between ultrasound findings and preterm infants with bronchopulmonary dysplasia (BPD).Methods: Preterm infants with a gestation age of less than 28 weeks or birthweight less than 1,500 g admitted to the neonatal intensive care unit (NICU) in the Chengdu Women's & Children's Central Hospital from June 2018 to June 2019 were enrolled in the study and divided into 2 groups: the BPD group and the non-BPD group. All clinical data and lung ultr… Show more

“…BPD can also be considered a common complication of RDS. The specific LUS pattern is based on inhomogeneous subpleural consolidations and fragment-like echoes, irregular pleural line, simultaneous presence of areas with normal parenchyma or compact B-lines, or white lung [90]. Considering that BPD occurs especially in the peripheral areas, a transabdominal approach is useful for the visualization of lung bases.…”

“…Alonso-Ojembarrena et al recently published a prospective trial further showing that LUS scores increased in the first week after birth and then decreased in infants who did not develop BPD, but that elevated scores in the second week predicted BPD (74% sensitivity, 100% specificity, AUC 0.93) (27). Gao et al have described LUS findings in infants with the diagnosis of BPD, including an abnormal pleural line, pulmonary consolidations with irregular hyperechoic areas, and prominence of B-lines (42).…”

“…A major limitation of neonatal LUS is the lack of a standardized acquisition protocol, including a lack of universally accepted guidelines for training, performing, and interpreting LUS. Most groups recommend utilizing lung zones for consistent assessment; however, there is discrepancy on how to divide the lung zones and how to assign the score (26,42). De Martino et al divides the lung into 6 zones (upper anterior, lower anterior, and lateral bilaterally) and then calculates a score of 0 to 3 based on aeration (26).…”

“…LU could also contribute in future to the development of BPD diagnostic criteria and the classification of its severity. Like chest X‐ray, LU could probably identify those infants with a more severely disrupted lung anatomy, but with a higher spatial definition 13 . This approach could identify cases at higher risk of future rehospitalizations, and the consequent need for preventive strategies, or those at higher risk of chronic obstructive pulmonary diseases who may need chronic medication.…”

This aspect should be investigated to help in the currently ongoing search for a comprehensive definition of BPD.…”
