Aim: To investigate the diagnostic performance of shear wave velocity (SWV) using virtual touch tissue quantification (VTQ) of acoustic radiation force impulse imaging (ARFI) technology in differentiating malignant and benign thyroid nodules by conducting a meta-analysis. Material and methods: The Cochrane library, Embase, Pubmed, and Web of Science were searched for relevant studies through December 2014. Studies evaluating the diagnostic accuracy of SWV in the identification of malignant and benign thyroid nodules by using VTQ of ARFI technology were selected. The cytology or histology was used as the reference standard. The pooled sensitivity, specificity, diagnostic odds ratio, likelihood ratio, and the area under the summary receiver operating characteristic (SROC) curve were used to examine the diagnostic accuracy of SWV. Results: A total of 13 cohort studies involving 1617 thyroid nodules from 1451 patients were identified. Of 13 studies, one was a retrospective study and others were prospective studies. The pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, and diagnostic odds ratio of SWV in differentiating malignant and benign thyroid nodules were 86.3% (95%CI: 78.2-91.7), 89.5% (95%CI: 83.3-93.6), 7.04 (95%CI: 4.40-11.26), 0.17 (95%CI: 0.10-0.31), and ), respectively. The area under the SROC curve was 94% (95% CI: 92-96). Conclusions: This meta-analysis indicates that VTQ is useful in evaluating the stiffness of thyroid nodules and differentiating between malignant and benign nodules. Due to the high sensitivity, specificity, and diagnostic odds ratio, SWV can be considered as a useful complement for conventional ultrasonography.

Recently ultRasonogRaphy equipment has remarkably improved. High-resolution ultrasonography excels in the detection of thyroid lesions, and it is an essential tool for diagnosing thyroid diseases. The present of specific features on B-mode sonography, evaluation of blood flow with Doppler sonography, and the strain ratio measured by strain elastography (SE) provide predictions of tissue characteristics to a certain degree. However, ultrasonography remains suboptimal for diagnosing thyroid diseases [1,2].The novel technology of shear wave elastography (SWE), also known as virtual touch quantification For phantom study, we measured the shear wave velocities (SWVs) of the four spheres of 2-and 1-cm diameters with varying hardness. For clinical study, the SWVs of normal thyroid glands and thyroid nodules, that were classified as benign or malignant according to either cytological or pathological findings, were measured. The SWVs of each thyroid patient were compared with that of a normal thyroid and each other. In phantom study, the SWVs for the 2-cm spheres correlated with the hardness of the targets, whereas the values for the 1-cm spheres did not. In clinical study, 112 nodules identified in 167 patients and 94 normal thyroid glands were analyzed according to the criteria for the study. The nodules included 84 benign nodules, and 28 papillary carcinoma. The mean SWVs of each group were 1.64 ± 0.47 m/s for normal thyroid, 1.88 ± 0.62 m/s for benign nodules and 2.67 ± 0.76 m/s for papillary carcinoma. The SWVs of papillary carcinoma were significantly higher than those of benign nodules (P < 0.001). The area under the ROC curve was 0.809 with a cutoff value of 2.01 m/s. The sensitivity and specificity were 85.7% and 62.0% respectively. Results showed that SWE provides new information on tumor characteristics, such as hardness and larger nodules tended to provide stable measurements.

Aim: To analyze the relationship between shear wave velocity in the kidney measured by point shear wave elastography using Virtual Touch Quantification (VTQ) (Siemens Acuson S2000) and histological parameters obtained from renal biopsies, in patients with chronic glomerulonephritis (CGN).Material and methods: The study group included 20 patients (mean age 47.95±13.59 years) with different types of CGN, that had underwent renal biopsy and 57 normal controls (mean age 38.07±17.32 years). In all patients, five valid stiffness measurements were obtained in each kidney, with the patient in lateral decubitus. Regarding the histological results, we assessed the presence or absence of glomerulosclerosis, interstitial fibrosis, and arteriolo-hyalinosis.Results: In patients with CGN we obtained the following mean values of VTQ values: right kidney: 2.12±0.81 m/s, left kidney 1.65±0.54 m/s, while in the normal controls significantly higher VTQ values were obtained: right kidney 2.69±0.72 m/s (p=0.004), left kidney 2.48±0.73 m/s (p<0.0001). In patients with CGN no statistically significant correlations between VTQ values and eGFR (r=0.37, p=0.12) or proteinuria (r=0.2, p=0.37) were found. We found significantly lower VTQ values in patients with interstitial fibrosis (1.46 vs. 1.99 m/s, p<0.05) and also in patients with arteriolo-hyalinosis (1.55 vs. 2.47 m/s, p<0.05).Conclusion: Our pilot study shows that shear wave velocity values in patients with CGN are significantly lower compared to normal controls, and there is a tendency to decrease with the decrease of eGFR, with the presence of interstitial fibrosis and of arteriolo-hyalinosis.
