The Advantages and Limitations of the Role of Core Needle and Fine Needle Aspiration Biopsy of Lymph Nodes in the Modern Era

Abstract: Fine-needle aspiration (FNA) has proven to be a rapid, cost-effective, and accurate means for evaluating a wide variety of conditions in most organ systems, although the FNA diagnosis of lymphoma (especially the primary diagnosis) remains controversial. However, recent changes to the World Health Organization classification of lymphomas place more emphasis on cytology and less emphasis on architecture, thus opening the door to a more expanded use of FNA, particularly in non-Hodgkin lymphomas. A review of the l… Show more

“…FNAC has a documented higher sensitivity in the diagnostic workup of metastatic malignancies which may be due to the fact that metastatic carcinoma cells are usually abundant and their cytologic features are dissimilar to that of the cells of normal or hyperplastic lymph nodes. 11,12 In present study, metastatic squamous cell carcinoma was found in majority of the cases (20 out of 30 cases, 66.7%). It included an interesting case of squamous cell carcinoma with granulomatous inflammation.…”

“…In case of metastatis, squamous cell carcinoma was found in majority of cases (30 out of 60 cases, 50%). Hirachand et al also noted the commonest type of metastic carcinoma to lymph nodes was of squamous cell variety [16]. Cervical lymph nodes, particularly high jugular and posterior cervical nodes, drain the head and neck and may harbour metastatic carcinomas originating in the nasopharynx, tonsilar fossa, tongue, floor of the mouth, thyroid, extrinsic larynx, facial skin and scalp.…”

“…Classical Hodgkin lymphoma can also resemble T-cell / histiocyte-rich large B-cell lymphoma or peripheral Tcell lymphoma [35]. For these reasons, fine needle aspiration cytology (FNAC) is considered less accurate for the diagnosis of Hodgkin lymphoma than it is for non-Hodgkin lymphoma [36].…”
