Aims: To study the efficacy of combination of extra-amniotic instillation of ethacridine with tablets Dinoprostone (PGE2) in second trimester abortion as compared to ethacridine alone. Methods: One hundred patients undergoing second trimester abortion were divided into two groups. Fifty patients received extra-amniotic ethacridine lactate 150 ml through Foley’s catheter (Ethacridine group) and the other 50 patients received 150 ml extra-amniotic ethacridine lactate mixed with a 0.5 mg of a crushed tablet of Dinoprostone through Foley’s catheter (Combination group). Both the groups were compared in respect to instillation-abortion interval, completeness of abortion and success rate. Results: The mean age, multiparity parity, and gestational age were comparable in both the groups. The instillation- abortion interval was significantly shorter in combination group as compared to ethacridine group (16.4±7.1 vs. 29.9 ±13.9 hours, p<0.001). Two patients in combination group had instillation abortion interval more than 48 hours as compared to 16 patients in ethacridine group(p<0.0005). Forty one patients (82%) in combination group had complete abortion as compared to 37 (74%) patients in ethacridine group. The success rate was 48/50(96%) in combination group as compared to 45/50 (90%) in ethacridine group. Conclusions: Addition of a 0.5 mg of crushed tablet of Dinoprostone in extra-amniotically instilled ethacridine lactate reduced the instillation-abortion interval significantly, increased the chances of complete abortion and increased the success rate. Background: Second-trimester pregnancy termination comprises 10 to 15 percent of the approximately 42 million abortions performed annually worldwide. Second-trimester procedures can be performed either with medication or by mechanical methods (Foleys catheter) or surgically by dilation and evacuation (D and E) or hysterotomy. The objective of the present study was to evaluate and compare the efficacy and safety of Tablet Misoprostol and extra amniotic infusion of normal saline with balloon catheter in second trimester abortion, to find out the maternal outcome, to compare the induction- to delivery interval between tablet misoprostol and extra amniotic infusion of normal saline with balloon catheter and to compare the cost of induction between tablet misoprostol and extra amniotic infusion of normal saline with balloon catheter.Methods: This randomized controlled study was carried out in the Obstetrics and Gynecology. Department of C.U. Shah Medical College, Surendranagar, from 23th August 2017 to 20th August 2018.Results: Amongst the 100 participants, 50 induction is done with tab. Misoprostol and 50 induction done with EASIBC. Success rate is higher in tab. Misoprost which is 90.20% and in EASIBC is 75.51%. There were longer induction abortion interval in EASIBC: Need for curettage among Misoprostol is 5 and EASIBC is 9. Major complication like PPH occurred in two cases in induction with tab. Misoprostol, which is managed by further intervention and no death occurred. No major complication occurred in induction with EASIBC.Conclusions: Excluding the side effects and complications, which are manageable induction with tab. Misoprostol is better than EASIBC in terms of induction abortion interval, effectiveness, acceptability.

Ethacridine lactate (EAL) is an antiseptic in solutions of 0.1%. It is also used as an agent for second trimester abortion. Two simple and sensitive spectrophotometric methods (Method A and Method B) were developed for the estimation of EAL in pharmaceutical formulations. Method A is based on the condensation of the primary aromatic amino group of ethacridine lactate with an aromatic aldehyde (p-Dimethylamino- cinnamaldehyde) to form a chromophore with absorption maximum of 600nm. Method B is based on redox reaction followed by complex formation of EAL with IO3-Metol reagent to form a stable chromogen, which can be estimated at 520 nm. Method A obeys Beer’s law in the concentration range of 2 to 12 μg/mL and method B in the range of 20 to 100 μg/mL. Interference studies were conducted to see the influence of excipients with the proposed methods. The common excipients usually present in dosage forms do not interfere in the proposed method A and method B. The optical characteristics, regression analysis data and precision of the methods were also calculated. The methods were validated for use in routine quality control of EAL in pharmaceutical formulations.
