Tension-Free Vaginal Tape (TVT) in Stress Incontinent Women with Intrinsic Sphincter Deficiency (ISD) – A Long-Term Follow-up

Abstract: In a prospective long-term study 49 women with stress incontinence and ISD (intrinsic sphincter deficiency) were followed for a mean of 4 years (range 3-5) after TVT operation. Preoperatively all patients underwent urodynamic investigations, quality of life evaluation, a 24-hour pad test and a gynecologic examination to properly verify the incontinence symptoms. The same protocol was used for postoperative evaluation. The mean age of the women was 66 years (+/- 11) and mean parity 2 (range 0-4). A standard TVT… Show more

“…For patients with recurrent incontinence, the results are similar to those achieved with a primary procedure; for those with a fixed urethrovesical junction, the outcome is poor, as it is with any other surgical procedures [41][42][43]. Holmgren conducted a long-term study, published in 2005, concluding that initial cure rates of TVT were good for mixed incontinence but did not persist after four years [44].…”

“…Midüretral sling operasyonlarının öğrenme eşiği ile ilgili literatürde net veri yoktur. Bizim hastanemiz ürojinekoloji bölümün-de, asistan vaka sayıları homojen olmadığı ve muhtemelen öğ-renme eşiği altında kaldığı halde majör komplikasyon oranları benzerlik göstermektedir [15,16]. Öte yandan, bilimsel camiada komplikasyonları bildirmeme eğilimi de, bizim minör komplikasyon oranlarımızın yüksek görünmesine sebep olmuş olabilir.…”

“…Bu nedenle ,literatüre katkı sağlaması amacıyla, kendi kliniğimizde asistanların uzman eşliğinde yaptığı midüret-ral slingleri komplikasyon ve tedavi oranları açısından inceledik. Midüretral sling operasyonlarında kanama insidansı tanıma bağlı değişmekle birlikte, TVT sonrası %0.7-8 arasında [14,15], TOT sonrası ise %0-2 arasında bildirilmiştir [16,17].Bizim çalış-mamızda, 91 TVT yapılan hastanın 3(%3.3) ünde transfüzyon gerektiren kanama izlenirken, 50 TOT yapılan hiçbir hastada transfüzyon gerektiren kanama izlenmedi. TVT yapılan ve transfüzyon gerektiren 3 hastadan birinde, postoperatif 4.saatte kanama nedeniyle laparotomi yapıldı.…”

“…This study was published in 1998 [9], but the results were available to the investigators at the beginning of the year before. The results of trials with a followup of 4 years on women suffering from recurrent stress incontinence, intrinsic sphincter deficiency, and mixed urinary incontinence were published in 2001, once again with excellent cure rates [10][11][12]. Already in the year 2000 5-year follow-up results were available and published in June 2001 showing an overall cure rate of 85 % according to the abovementioned criteria [13] ( Table 1).…”
