“…The safety of this "watch and wait" approach would appear to be supported by both the survival curves in the SCD-HeFT and MADIT-II trials which did not separate until after 12 months post ICD insertion [18]. Our results have further support, as the benefit of ICD has been shown to be less pronounced in non-ischaemic cardiomyopathy patients and those with an LVEF between 31 and 34% [4].…”

“…On the contrary, this category of patients has been included in the prophylactic ICD studies. Interestingly, in the Multicenter Automatic Defibrillator Implantation Trial II (MADIT-II) and the Sudden Cardiac Death in Heart Failure (SCD-HeFT) trials it was observed that patients with QRS >150 or >120 ms, respectively had greater benefit from an ICD compared to patients with narrow QRS complex [24, 34]. Hence, this observation may support the indication of addition of an ICD in symptomatic patients with wide QRS complex who are planned for a CRT implantation.Conversely, it can be argued that implantation of CRT could be sufficient in these patients as the CARE-HF study showed a significant reduction in sudden cardiac death as well.…”

“…In the Multicenter Automatic Defibrillator Implantation Trial -II, implantation of an AICD more than 30 days postinfarction was associated with a significant reduction in sudden cardiac death and total mortality [61]. There was no apparent benefit of AICD implantation on total mortality for the first year postimplantation.…”
