Technical guidelines for head and neck cancer IMRT on behalf of the Italian association of radiation oncology - head and neck working group

Abstract: Performing intensity-modulated radiotherapy (IMRT) on head and neck cancer patients (HNCPs) requires robust training and experience. Thus, in 2011, the Head and Neck Cancer Working Group (HNCWG) of the Italian Association of Radiation Oncology (AIRO) organized a study group with the aim to run a literature review to outline clinical practice recommendations, to suggest technical solutions and to advise target volumes and doses selection for head and neck cancer IMRT. The main purpose was therefore to standardi… Show more

“…The entity of this uncertainty depends on several factors, such as the type and quality of images used for delineation, the experience and professional background of observers, and the existence of tools to help and drive the contouring process. Contouring guidelines can lead to a higher precision in larynx delineation [9,20], as well as a reduction in the interobserver variability with consequent implications on the dose distribution to OARs. Moreover, atlases of radiologic anatomy, case examples, and contouring courses could further improve the accuracy in volume delineation.…”

“…Concerns have been raised recently regarding long-term mortality of concurrent chemoradiation, while treatment regimens employing induction chemotherapy and cetuximab are presently under investigation. Finally, radiotherapy has a crucial role in the combined modality treatment of thoracic malignancies and head and neck cancers (106). Thus, young radiation oncologists will need to acquire a multidisciplinary profile in terms of knowledge, skills, and attitude to be able to properly fit into the multimodality bent of modern clinical oncology.…”

“…Each patient underwent PET/ CT acquisition at least 60 min after 18 F-FDG injection, following good hydration. All the patients had fasted for at least 6 h and at the time of 18 F-FDG administration, blood glucose levels were below 150 mg dl 21 . Injected activity was 3.7 MBq/kg.…”
