Delineation of the larynx as organ at risk in radiotherapy: a contouring course within “Rete Oncologica Piemonte-Valle d’Aosta” network to reduce inter- and intraobserver variability

Abstract: Mean percentage of intersection and DC strongly increased after the course, rising from 57.76 to 93.83 % and from 0.68 to 0.89, respectively. CI enhanced from 0.06 to 0.31. Conclusions This study shows an improvement in larynx definition after the contouring course with lower interobserver variability and major consistency compared to the reference volume. Other specific educational activities may further increase the quality of radiation therapy contouring in this setting.

“…Apart from the introduction of atlas-based auto-contouring programs in the clinical practice, which proved to be beneficial in time reduction compared to the manual contouring [26], the possibility of expanding the role of CSRTs in the pre-planning phase represents a major asset. Several studies have already confirmed the great potential of CSRTs in normal tissue contouring in the head and neck and the pelvic districts [10][11][12][13]27].…”

“…In this article, the mean volume refers to the average volume delineated by the five observers. Conformity index (CI) and Dice coefficient (DC) were used in articles with similar research methods, which can be used to express the consistency of delineation between observations ( 15 ). The mean volume, CI, and DC of each sequence were compared to determine the variability between observers.…”

“…Pre-operative RT is a well-established option to provide tumor downsizing and downstaging and to increase loco-regional control in this setting [3] . Selection and delineation of clinical target volume (CTV) and organs at risk (OARs) are crucial steps to deliver precise and tailored radiation [4] , [5] , [6] . While the sites and subsites of irradiation are, to some extent, agreed by physicians, the boundaries of CTVs still remain controversial, leading to inhomogeneous contours and systematic errors with standard deviations as high as 1 cm, as reported in different studies [7] , [8] .…”
