TCRβ Clonality Improves Diagnostic Yield of TCRγ Clonality in Refractory Celiac Disease

Abstract: The combined analysis of both rearrangements allowed recognition of monoclonal populations in otherwise negative patients, with detection rates from 20% (Vγ only) to 33% (Vγ and Vβ), thus raising the likelihood of early identification of RCD patients at high risk of death.

“… 5–8 Strategies to diagnose RCD type II include immunohistochemistry revealing a loss of CD8 in intraepithelial lymphocytes (IELs) in combination with evidence for clonal expansion of IELs, as uncovered by T-cell receptor β (TCRβ) or TCRγ-PCR in duodenal samples. 9 10 Alternative diagnostic procedures include flow cytometric immunophenotyping of isolated IELs from duodenal mucosa. 11 …”

“…Analysis of the delta chain rearrangement may be useful for diagnosis of RCDII in patients presenting oligoclonal rearrangement of the gamma chain [22]. Others have suggested interest of detecting of the beta chain of the TCR [31]. Finally, specificity of the PCR product needs to be attested by formation of homoduplexes [22].…”

“…Analysis of the delta chain rearrangement may be useful in RCDII patients presenting oligoclonal rearrangement of the gamma chain [7]. Interest of detecting the beta chain of the TCR has also been suggested [20]. Nevertheless, evidence that abnormal IELs originate from immature T cells tempers interest of studying beta rearrangement which occurs later in the ontogeny.…”
