Tc-99m Sestamibi Before and During Treatment in a Patient With Sarcoidosis and Persistent Hyperparathyroidism

Abstract: This decrease of MIBI uptake in the mediastinal lymph nodes with therapy may imply that MIBI can be used to assess the response to treatment in sarcoidosis. Perhaps a relation exists between MIBI uptake at the moment of diagnosis and prognosis. Further studies in more patients are needed to evaluate the role of MIBI in the management of sarcoidosis.

“…13 We report a case of cervical lymph node sarcoidosis misdiagnosed as parathyroid adenoma. Accumulation of MIBI in hilar lymph nodes and in pulmonary interstitial fibrosis in sarcoidosis has been reported previously; [14][15][16] other cases of sarcoidosis have been misinterpreted as primary hyperparathyroidism before the MIBI was introduced into clinical practice. 12,13 A case of active sarcoid mimicking a mediastinal parathyroid adenoma on MIBI scan was previously described by Klieger.…”

“…MIBI накапливается и в гиперметаболических узлах ЩЖ, опухолях из окси-фильных клеток, метастазах рака ЩЖ, раке молоч-ной железы, а также в лимфатических узлах при ту-беркулезе и саркоидозе [20][21][22][23][24][25]. Учитывая разное накапение MIBI при саркоидозе до начала и в про-цессе лечения, предлагалось использовать данный метод для оценки эффективности терапии [26].…”

“…Malignant lymphadenopathy has been shown to accumulate 99m Tc‐MIBI in metastatic melanoma and breast, thyroid, or pelvic malignancy. The 99m Tc‐MIBI is also taken up by mediastinal lymph nodes in sarcoidosis 26–30 . Cutaneous KS is often complicated by venous stasis of the extremities, face and genitals.…”
