Tadalafil once daily in the management of erectile dysfunction: patient and partner perspectives

Abstract: Erectile dysfunction (ED) is a prevalent condition that affects men and their partners. Significant improvements in the sexual lives of these couples have been achieved with the introduction of phosphodiesterase 5 (PDE5) inhibitors. A PDE5 inhibitor is now widely recognized as the first-line therapy for the majority of men with ED. Currently, three PDE5 inhibitors – sildenafil, tadalafil and vardenafil – are approved to be taken as needed in anticipation of sexual activity, but only one of these, tadalafil, ha… Show more

“… 5 6 7 8 While phosphodiesterase type-5 (PDE5) inhibitors are effective on-demand, they require time after administration to achieve full efficacy, which may reduce spontaneity in sexual activity. 9 10 Pharmacokinetic analyses demonstrate that the 17.5-h half-life of tadalafil allows for once-daily dosing, with stable serum concentrations attained within 5 days. 11 Under steady-state conditions, tadalafil exposure is 1.6-fold more than that after a single dose, which may increase the rate of obtaining an erection upon sexual stimulation.…”

“…In addition, many patients and their partners dislike the necessity of on‐demand treatment for ED. Intermittent dosing may not be an optimal treatment for all men and their partners based on individual patterns of sexual activity . In 2008, a change in the standard of management of ED occurred with the approval of once‐daily dose of tadalafil by the US FDA for the management of ED.…”

“…In 2008, a change in the standard of management of ED occurred with the approval of once‐daily dose of tadalafil by the US FDA for the management of ED. The prolonged elimination half‐life of tadalafil of approximately 18 h has the advantage of separating medication from sexual interactions and may restore sexual spontaneity .…”

“…20 The approval of once-daily tadalafil signaled a paradigm shift in ED management. 21 In clinical trials, once-daily tadalafil improved erectile function and sexual satisfaction (vs placebo). [22][23][24][25][26][27][28] However, none of these studies evaluated the efficacy and safety profiles of once-daily tadalafil specifically in men with mild (vs moderate) ED or aged X50 (vs o50) years.…”
