Systematic review of the oncological and functional outcomes of pelvic organ‐preserving radical cystectomy (RC) compared with standard RC in women who undergo curative surgery and orthotopic neobladder substitution for bladder cancer

Abstract: For well-selected patients, POPRC with orthotopic neobladder may potentially be comparable to standard RC for oncological outcomes, whilst improving sexual and urinary function outcomes. However, in women undergoing RC, oncological and functional data regarding POPRC remain immature and require further evaluation in a prospective comparative setting.

“…Attempted bilateral or unilateral NS compared with no attempted NS positively influenced daytime continence in patients who underwent SC, whereas nighttime continence rates did not differ significantly among the three groups because of the similar continence rates between the bilateral and unilateral group. Our day-and nighttime continence rate of 87.5% is in line with the results from other genital organ-sparing case series, with daytime and nighttime continence rates of 64-100% and 50-89%, respectively [2].…”

“…Incomplete emptying or 'hypercontinence' is another complication of OBS in women, with reported rates of ISC ranging from 0% to 31% [2]. In the present study, the rate of ISC was the same in both groups and the small number of patients did not allow further analysis of the cause of incomplete emptying.…”

“…In this setting, when an ON is performed, the procedure can be associated with a considerable rate of voiding symptoms [4,5]. In addition, sexual dysfunction derived from such a highly demolitive surgical procedure is a key concern, especially in younger patients due to a significant impact on health-related quality of life (HRQoL) [6].…”
