Objectives: This study is part of the development of an International Classification of Functioning, Disability and Health (ICF) Core Set for spinal cord injury (SCI). Its specific objectives were to identify outcome parameters reported in published studies on individuals with SCI in the early post-acute and chronic situation, and to identify and quantify the concepts of the reported parameters using the ICF as a reference. Methods: Electronic searches of Medline, EMBASE, PsycINFO and CINAHL from 2001 to 2005 were carried out. All outcome parameters and their underlying concepts were retrieved from the included studies. These concepts were linked to categories of the ICF using standardized rules. Results: From the 6681 abstracts retrieved, 2205 were randomly selected (33.0%) and 281 studies met the inclusion criteria (12.7%). A total number of 5217 concepts were retrieved from standardized and non-standardized measures, of which 4049 (77.6%) could be linked to 175 different ICF categories: 56 out of 114 Body Functions, 19 out of 56 Body Structures, 62 out of 118 Activities and Participation and 38 out of 74 Environmental Factors categories. Second-level categories reported in 420% of all studies were pain, remunerative employment, health services, systems and policies, school education and higher education. Conclusion:The ICF provides a valuable reference to identify and quantify the concepts of measures focusing on SCI in the early post-acute and chronic situation. The findings show a great diversity in the consequences of SCI and underscore the importance of social participation and environment for people with SCI.

After linking the two instruments to the ICF core set for stroke, data were analyzed and presented in terms of activityspecific domain-scores for the total sample and sub-groups according to gender and age.Results: Together the two instruments covered 69 % of the Activities and participation component of the ICF core set for stroke. Two activity-specific domains were identified within the BI and six within the mFAI. Most participants reported a high overall activity level. Inactivity was most common among those ≥80 years. Men and women participated in different types of activities and used different modes of transport.Conclusions: Long-term stroke survivors have a high activity level in daily life, though individual variation is considerable. The structure provided by linking instruments to the ICF core set for stroke can be used for more fine-tuned descriptions of activity.3

Einleitung ! Die Therapie akuter Erkrankungen in der Klinik orientiert sich an der Diagnose und den speziellen Bedürfnissen des Patienten. Dabei haben besonders Patienten mit akuten und chronischen Begleiterkrankungen und ältere ein erhçhtes Risiko für Komplikationen und Verlust der Funktionsfähigkeit. Daher ist es besonders wichtig, Zusammenfassung ! Hintergrund: Im Akutkrankenhaus spielen Physiotherapeuten eine Schlüsselrolle bei der Behandlung der Funktionsfähigkeit akut erkrankter Menschen. Um die funktionsorientierten Ziele der physiotherapeutischen Intervention festzulegen und die Ergebnisse interdisziplinär zu kommunizieren, sind spezifische Messinstrumente notwendig, die in einem fach-und berufsübergreifenden Rahmen die Patientenprobleme darstellen. Die ICF kçnnte diesen Rahmen bieten, muss aber in eine passende Form gebracht werden. Ziel: Die Ausprägungsmerkmale von für die Physiotherapie relevanten ICF-Kategorien werden so definiert, dass Physiotherapeuten damit im Akutkrankenhaus beurteilbare Patienteneigenschaften beschreiben kçnnen (operationalisieren). Methode: 6 Experten operationalisierten 124 ICF-Kategorien durch einen formalen Entscheidungs-und Konsensusprozess. Anschließend prüften 6 Physiotherapeuten deren Nutzbarkeit an 28 Patienten. Ergebnisse: Auf einer ordinalen Skala wurden 30 und auf einer dichotomen Skala 94 ICF-Kategorien operationalisiert. Die Anwendung der operationalisierten ICF-Kategorien am Patienten erwies sich als unproblematisch. Schlussfolgerungen: ICF-Kategorien kçnnen für Physiotherapeuten nutzbar operationalisiert werden. Für die Entwicklung eines validen und reliablen Messinstruments sind weitere Untersuchungen notwendig.Abstract ! Background: In emergency hospitals' settings physical therapists play a key role in the treatment of patients' loss of functioning. In order to set the functioning-oriented goals of physical therapy intervention and communicate results of treatment in a multidisciplinary environment, specific assessments are needed. These instruments should depict the patients' problems through a framework that overlaps all health care professions involved in the treatment. The ICF is a multi-purpose classification which may be the appropriate basis for this assessment but needs adaptation. Objective: Items based on ICF categories relevant to physiotherapeutic interventions are defined so that physical therapists in emergency hospitals can describe patients' characteristics (operationalisation). Method: 6 experts operationalised a selection of 124 ICF categories in a formal decision-making consensus process. Afterwards 6 physiotherapists tested the practicability of the operationalised ICF categories on 28 patients. Results: On an ordinal scale 30 and on a dichotomous scale 94 ICF categories could be operationalised. In using the operationalised ICF categories no major problems were reported.
