Systematic Review and Meta-Analysis of Linezolid versus Daptomycin for Treatment of Vancomycin-Resistant Enterococcal Bacteremia

Abstract: cLimited therapeutic options exist for the treatment of vancomycin-resistant Enterococcus (VRE) bacteremia; the most commonly used are daptomycin and linezolid. We attempted a systematic review and meta-analysis of the comparative efficacy of those two agents. Studies comparing daptomycin to linezolid treatment for VRE bacteremia, published until August 2012, were identified from the MEDLINE, EMBASE, CENTRAL, ISI Web of Science, and SCOPUS databases. All comparative studies on patients older than 18 years of a… Show more

“…Enterococci constitute the second most common cause of nosocomial urinary tract infections [2]. This species has been recognized as the third leading cause of healthcare-associated bloodstream infections in the USA, generating an enormous cost burden for the healthcare system [3]. Enterococci have gained resistance to almost the entire antimicrobial spectrum, including vancomycin, undoubtedly the most potential antimicrobial agent as a treatment for enterococcal infections [4].…”

“…In 2004, VRE strains were initially reported in Iran; and presently pertain to be a highly relevant nosocomial pathogen [5,6]. It is quite significant to mention that VRE has been identified as a global public health dilemma, as the therapeutic options for the treatment are utterly limited [3]. Infections due to VRE are significantly associated with an increased rate of morbidity, mortality, prolonged hospitalization, and elevated cost burden [7].…”

“…Recently published meta-analyses revealed increased survival with linezolid compared to that with daptomycin (18,19). One explanation for the differences in the findings in these studies compared to the current one is that these prior studies included In the financial data cohort, 33 (91.7%) subjects within the daptomycin group were matched to 33 (31.4%) within the linezolid group; 23 (63.9%) subjects in the daptomycin group were matched to 23 (41.8%) in the ␤-lactam group; and 46 (43.8%) subjects in the linezolid group were matched to 46 (83.6%) subjects in the ␤-lactam group.…”

“…Balli ve ark. 'nın (29) yaptığı meta analizde ise VRE bakteriyemisinde linezolid tedavisinin daptomisine göre daha etkili olduğu ve mortalite oranlarının daha düşük olduğu belirtilmesine karşın, linezolidin kısmen hematolojik yan etkilerinin olması uzun süreli kullanımını kısıtlamaktadır.…”
