Sympathetic ophthalmia after surgical resection of iridociliary melanoma. A case report

Abstract: Sympathetic ophthalmia is a rare, but severe disease that can occur after resection of iridociliary melanoma. High-dose steroid therapy and supplementation with immunosuppressive agents early in the course of the disease was effective in resolving the condition.

“…In many cases of sympathetic ophthalmia, however, the choriocapillaris is spared, leaving clinical history as the only means of differentiating the 2 entities. 11 Meningeal symptoms may include meningismus, vertigo, ataxia, sensorineural hearing loss, 12,13 and rarely, a cerebral vasculitis. 14 Sympathetic ophthalmia may be prevented if the injured eye is enucleated soon after the inciting event; however, this is not routinely done, especially if the injured eye has visual potential.…”

“…[3][4][5][6][7][8][9][10] Contemporary literature has hypothesized an association between surgical resection or radiotherapy for choroidal melanoma and subsequent development of SO. [11][12][13][14] The exact pathophysiology of SO remains unknown; however, it does appear to be an autoimmune hypersensitivity reaction against exposed ocular antigens in the inciting eye as well as the sympathizing eye. 1,2 We report an unusual case of SO prompted by an untreated choroidal melanoma in the fellow eye.…”

“…[3][4][5][6][7][8][9][10] Choroidal melanoma treatment strategies, including radiotherapy, brachytherapy, and surgical excision, have been reported as "stimulating" factors for SO in the current literature. [11][12][13][14] The presumed exposure to uveal antigens during these procedures is postulated to be the likely etiology.…”
