Suture granulomas developing after the treatment of oral squamous cell carcinoma

Abstract: HighlightsA 64-year-old man underwent head and neck surgery for oral squamous cell carcinoma.Multiple suture granulomas mimicking tumor recurrence developed in the radiation field. The suture granulomas developed shortly after adjuvant chemoradiation therapy.Suspect suture granulomas when nodules appear during postoperative radiotherapy.Suture granulomas may show spontaneous resolution.

“…FDG-PET/CT is the most sensitive non-invasive imaging method for the detection of tumor metastasis and recurrence, but sometimes reveals false-positive results [16] . In various organs, there were several false-positive case reports of suture granuloma by FDG-PET /CT developed at surgical margin of neoplasms in the lung [ 4 , [16] , [17] , [18] , [19] , [20] , [21] ], in the oral cavity [22] , in the thyroid [ 23 , 24 ], and in the neck [ 25 , 26 ], in the stomach [ 27 , 28 ], in the colon [ 8 , 9 , 29 ] and in the ovary and testes [ 15 , 30 ]. It should be emphasized that suture granulomas can show false-positive findings on FDG-PET/CT, thus requiring differential diagnosis from recurrent tumors [26] .…”

“…• Mainly after ointment dressings (eg, iodine allergy) or local antibiotic ointments • Prescribe local cortisone and antihistamines 7 Late complications (weeks/months after surgery) Suture granuloma/suture dehiscence • Surgical removal of granuloma or suture • Braided or polyfilament sutures may more commonly lead to infection and granuloma formation; if applicable, reduce use in auricular region 26 • Do not remove nonresorbable sutures prior to 10 days to avoid dehiscence • Do not place cutaneous sutures under maximum tension (increased risk of dehiscence) 7…”

“…In addition, while there are not many publications in the literature about suture-related non-healing wounds, granulomas that usually develop due to suture and manifest cancer-like findings are discussed. 8,9 Interestingly, our four patients developed excessive reaction due to prolene suture. A deeper understanding of the interaction between the material and the biological environment is essential to overcome side effects and to develop strategies and solutions in the use of these materials, which are still a major challenge in the biomedical field.…”

“…Although suture‐related chronic reactions are mostly seen after silk sutures, they do not occur with absorbable and monofilament sutures. In addition, while there are not many publications in the literature about suture‐related non‐healing wounds, granulomas that usually develop due to suture and manifest cancer‐like findings are discussed . Interestingly, our four patients developed excessive reaction due to prolene suture.…”
