Background Telehealth technology and related products can help to solve the problems associated with providing health care services and equal distribution of resources. However, in order to run a telehealth business successfully, key components and critical factors need to be taken into account. A telehealth business framework can provide a rich understanding of these components and factors. Therefore, the present study aimed to identify the key components and critical factors for developing a telehealth business framework from the experts’ perspectives. Methods The present qualitative study was conducted in 2020. The participants were 22 experts in the fields of medical informatics, health information management, telemedicine, telehealth, health entrepreneurship, health insurance, and digital health start-ups. In depth semi-structured interviews were conducted to collect data, and the data were analyzed using framework analysis. Results Four main themes derived from data analysis. The themes included key components for developing a telehealth business framework, success factors, challenges, and barriers of a telehealth business. Overall, the results indicated that the key components in a telehealth business framework included created value, key resources, key activities, key partners, licenses and permissions, product pricing, revenue, marketing, supporting services, and getting feedback from customers. Although receiving support from different individuals and organizations as well as economic benefits of telehealth services may consider as success factors, there are a number of challenges and barriers which should be taken into account. Otherwise, these factors may hinder the success of a telehealth business. Conclusions The development of a telehealth business framework might be an important step towards developing a more complete business plan, facilitating the commercialization of telehealth products, and providing a solution for product sustainability in a competitive market. In the current study, the key components and critical factors for developing a telehealth business framework were identified; however, further research is needed to explore how these components and factors can be helpful in developing business plans and running a successful telehealth business.

Objectives With telemedicine becoming more widely implemented in emergency situations, understanding the quality and content of current findings that explore prehospital telemedicine is vital to establish best practices and guide future research. This systematic review examines the clinical importance of telemedicine in patient-provider ambulance-based settings with a focus on multifunctional systems for general prehospital emergency populations. Methods Using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses methodology we found 1564 abstracts, which were blind-reviewed by independent reviewers. Relevant articles were reviewed, classified, and analyzed according to research methods and technology type, as well as quality, cost, and satisfaction. The studies were also reviewed for validated evidence-based practice. Results Those studies that looked at cost, quality, and satisfaction with care generally shared favorable results. Setup notwithstanding, cost was comparable or less than controls. Care quality was also found to be in line with or slightly preferable to face-to-face care with some advantages in response time and quality. Patients and providers were satisfied with the systems. Common obstacles included limited bandwidth and small sample sizes. Conclusions Although feasibility remains salient, research regarding the impact of ambulance-based telemedicine on patients and healthcare providers is encouraging, but nascent. As a whole, this body of literature does not yet adequately speak to the most important concerns of medicine: quality, cost, and satisfaction. More research is needed in each of these areas. However, those studies that do address these matters share hopeful results. Future research should test these mechanisms in prehospital settings with greater rigor.

Telehealth, the interactive, electronic exchange of information for diagnosis, treatment, support, or care management, is a critical component of many systems of care.1 The variety of telehealth uses, technologies, and approaches, when implemented successfully, can improve access to care and overcome typical geographic and workforce barriers.1 The opioid epidemic and recent social distancing requirements due to coronavirus disease (COVID-19) have prompted public safety, behavioral health, and other community partners to explore how telehealth can be used for identification and treatment of disorders in the criminal justice system. Telehealth offers an innovative strategy for intervention and treatment within the criminal justice system, which is greatly needed given the prevalence of justice-involved populations affected by opioid-use disorder. For one, it is costly to provide treatment and care to people who are incarcerated, and telehealth can help reduce that cost.2 Using telehealth to connect these people to providers can also help avoid resource-heavy and potentially dangerous transfers to care settings. In addition, a telehealth strategy can minimize the burden associated with recruiting and retaining healthcare providers on site, particularly behavioral health providers, who are often in short supply. Offering options virtually is one way to ameliorate organizational challenges. Services that can be provided via telehealth include inpatient and outpatient medical services, behavioral health, physical and occupational therapy, disaster management, health education,1 and coordination of care support for inmates, providers, and officials. Telehealth can also be used to facilitate release of inmates by connecting people who are in prison or jail with community-based services before release.

Abstract: We concluded that, although the first four approaches can contribute to the success of a center, telehealth centers are and should remain cost centers for their respective institutions.
