«Suspects» in Etiology of Endemic Nephropathy: Aristolochic Acid versus Mycotoxins

Abstract: Despite many hypotheses that have been challenged, the etiology of endemic nephropathy (EN) is still unknown. At present, the implications of aristolochic acid (AA) and mycotoxins (ochratoxin A—OTA and citrinin—CIT) are under debate. AA-theory is based on renal pathohistological similarities between Chinese herbs nephropathy (CHN) and EN, findings of AA-DNA adducts in EN and in patients with urinary tract tumors (UTT), as well as the domination of A:T®T:A transversions in the p53 mutational spectrum of UTT pat… Show more

“…Aristolochic acids (AAs), naturally occurring nephrotoxicants and human carcinogens, are associated with the development of a rapidly progressive interstitial nephritis called aristolochic acid nephropathy (AAN) [45], and may also be a cause for the Balkan Endemic Nephropathy (BEN) [46]( Figure 6). AAs are a mixture of structurally related nitrophenanthrene carboxylic acids [47].…”

“…For example, studies conducted in Croatian and Bulgarian endemic nephropathy (EN) areas were focused on the levels of resident exposure to OTA (2,3). Cereals are commonly contaminated with the Fusarium species and therefore often analysed for the presence of Fusarium mycotoxins such as FBs, ZEA, deoxynivalenol (DON), diacetoxiscitpenol (DAS), beauvericin (BEA), or T-2 toxin (4-6), but only a limited number of studies have paid attention to or specifi ed the percentage of co-contaminated samples.…”

“…Aristolochic acids (AAs) are naturally occurring nephrotoxicants and human carcinogens that are associated with the development of a rapidly progressive interstitial nephritis called aristolochic acid nephropathy (AAN) (Debelle et al, 2008), and they may also be a cause of Balkan endemic nephropathy (BEN) (Pepeljnjak and Klaric, 2010). Aristolochic acids are a mixture of structurally related nitrophenanthrene carboxylic acids (Kumar et al, 2003).…”
